摘 要:目的:探讨不同序列在乳腺MR成像中的应用价值。方法:对10名健康志愿者和42名乳腺疾病患者分别采用FSE-IR、FSE-XL T1WI、FSE-XL T2WI和3D-FSPGR序列进行乳腺MR成像,从图像信噪比、对比噪声比、病灶显示数目等方面对其应用价值进行评价。结果:抑脂条件下,FSE-IR和FSE-XL T2WI图像信噪比和对比噪声比明显高于FSE-XL T1WI,差别均有显著性意义(P<0.05);FSE-IR序列图像对比噪声比高于FSE-XL T2WI,差别有显著性意义(P<0.01),两者之间的信噪比差别无显著性意义(P>0.05)。42例患者采用上述序列联合检出45处病变,其中恶性3例,共5处病变;良性34例,共40处病变。不同序列对病变的检出率分别为FSE-IR93.33%(42/45)、FSE-XL T2WI93.33%(42/45)、FSE-XL T1WI42.22%(19/45)、3D-FSPGR T1WI动态增强为82.22%(37/45)。结论:3.0TMR乳腺成像,联合使用FSE-IR、非抑脂FSE-X LT1WI序列横轴面、抑脂FSE-XL T2WI矢状面平扫和3D-FSPGR动态增强,可在较短的扫描时间内获得优质的扫描图像,提高对病变的检出率。Objective: To assess the value of different sequences in breast magnetic resonance imaging. Methods: Ten healthy volunteers were examined with FSE-IR, fat suppressed FSE-XL T2 WI,and FSE-XL T1 WI with or without fat sup pression respectively in axial view, and the SNR and CNR of the images acquired by different sequences were compared. Forty two patients received FSE IR and non fat suppressed FSE X1TI WI in axial view,FSE-XL T2WI with fat suppression in sagittal view and 3D-FSPGR for dynamic contrast-enhancement in axial view. Then the applied value of above sequences was evaluated by the number of detected lesions. Results: Compared by SNR and CNR among the three fat suppressed sequences, both FSE IR and FSE-XL T2WI offered higher SNR and CNR than FSE-XL T1WI, and the results showed statisti cally significant difference (P〈0.05). The CNR of FSE-IR was higher than that of FSE-XL T2WI, and there was statisti eally significant difference (P〈0.01), but there was no significant difference between their SNR (P〉0.05). There were 3 cases having 5 malignant breast lesions and 34 cases having 40 benign lesions, and the remaining 5 cases showed no abnor reality. The detection rate of the different sequences for breast lesions was 93.33% (42/45) by FSE-IR and FSE-XI. T2 WI, 42.22% (19/45) by FSE-XL T1WI,and 82.22% (37/45) by 3D-FSPGR respectively. Conclusion:Combination of FSE-IR, non fat suppressed FSE-XL T1WI in axial view, fat suppressed FSE-XL T2WI in sagittal view and contrast-enhanced dy namic 3D-FSPGR in axial view at 3.0T for breast MRI can obtain excellent imaging quality in short scanning time and improve diagnostic accurate rate.
