注射用碳酸酰胺过氧化氢在开胸单肺通气时的应用

摘　　要：目的注射用碳酸酰胺过氧化氢（下称碳胺氧）对开胸单肺通气时低氧血症的防治效果。方法36例择期开胸行单肺通气病人，年龄33-82岁，ASAⅠ～Ⅱ级。随机均分为两组，Ⅰ组为对照组，Ⅱ组为观察组。单肺通气后以5ml／min速度分别滴入5％葡萄糖液300ml（Ⅰ组）和溶于5％葡萄糖液300ml中的碳胺氧lg（Ⅱ组）。术前及单肺通气后20、40、60、80min各时点采动脉血行血气分析。结果Ⅱ组与Ⅰ组相比，单肺通气且给予碳胺氧20、40、60min时，PaO2和SpO2均升高，有显著性差异（P〈0．01和〈0．05），pH和PaCO2无显著性差异（P〉0．05）。Ⅱ组内各值与术前相比，单肺通气40、60、80min时的PaO2和SpO2都升高，差异有显著性（P〈0．01和〈0．05）。结论注射用碳酸酰胺过氧化氢用于开胸单肺通气时，是防治低氧血症的有效措施之一。Objective The study was designed to determined the effect of carbonyl hydrogen peroxide to prevent and cure the hypoxidosis during the period of one lung ventilation in thoractomy. Methods 36 patients scheduled for thoracic surgery , aged 33-82, ASA Ⅰ - Ⅱ, served as the experimental subjects .Anesthesia was induced with propofol 2mg/kg , fentanyl 2 μg/kg , and succinylcholine chloride 2mg · kg^-1.The patients were divided into two groups at random .The patients in group Ⅰ were given 5% liquid glucose at the rate of 5ml/min , and those in group Ⅱ were given lg carbonyl hydrogen peroxide that was dissolved in 300ml 5% liquid glucose at the same rate.All the patients were drew arterial blood for a blood gases analysis at preoperation and 20, 40, 60and 80min after one lung ventilation. Results The PaO2 and SpO2 in group Ⅱ, which were given carbonyl hydrogen peroxide, were higher than those in groupⅠ after one lung ventilation 40, 60and 80min （ P〈0.01and 0.05 ）, but pH and PaCO2 were not significantly different. The PaO2 and SpO2 in group Ⅱ after lung ventilation 40, 60and 80min were higher than those preoperation （ P〈0.01and 0.05 ） . Conclusion Carbonyl hydrogen peroxide can effective prevent and cure hypoxidosis during the period of one lung ventilation in thoractomy.
