Orem自理理论在CO2激光辅助下人工镫骨植入术后护理中的应用

摘　　要：[目的]探讨Orem自理理论在CO2激光辅助下人工镫骨植入术护理中的应用效果。[方法]将80例耳硬化症病人随机分为对照组和实验组,对照组按术后常规进行护理,实验组在此基础上应用Orem完全补偿系统、部分补偿系统和支持教育系统进行护理干预。观察两组术后焦虑、抑郁情况,以及下床活动时间、住院时间、医疗费用情况。[结果]与对照组比较,实验组术后下床活动时间明显提前(P<0.01),术后病人焦虑、抑郁评分明显降低(P<0.05),住院时间缩短,医疗费用降低,护理满意度提高(P<0.05)。[结论]将Orem自理理论用于人工镫骨植入术后护理中,能调动和激发病人的主观能动性,提高自我护理能力。Objective： To probe into the effect of applying Orem self- care theory in postoperative nursing care of patients accepting artificial stapes implantation assisted by CO2 laser. Methods. A total of 80 cases with otosclerosis were randomly divided into control and test group. Patients of control group were carried out postoperative care as in routine. Patients of test group were given nursing intervention by applying Orem fully compensation system,partial compensation system and support education system besides the routine nursing care. The anxiety, depression state after the operation, the time of off - bed activity, length of stay, and medical costs of the two group cases were observed. Results：Compared with control group, the time of off- bed activity in patients of test group was significantly earlier than that of control group cases （P〈0. 01）. The scorings of postoperative anxiety and depression test group patients were significantly lower than that of control group cases （P〈0.05）. And length of stay of test group cases were shortened, medical costs of them were decreased, and nursing satisfaction of them were enhanced compared with that of patients of control group （P〈0.05）. Conclusion：Orem self- care theory for postoperative nursing care of patients accepting artificial stapes implantation can mobilize and motivate the subjective initiative of patients. It can enhance the ability of self- care of patients as well.
