摘　　要：目的:研究胃癌组织中类肝素酶(Heparanase,HPSE)的表达与临床病理特征及生存期的关系,探讨其作为胃癌预后指标的可行性。方法:应用免疫组化方法检测具有随访资料的103例胃腺癌标本中HPSE的表达,并将检测结果与临床病理特征进行综合分析。结果:(1)103例胃癌组织中HPSE的阳性表达率为57.3%(59/103),在胃正常组织中均无表达;(2)HPSE的表达与患者的性别、年龄、肿瘤大小、部位和分化程度均无相关性(P>0.05),而与肿瘤的浸润深度、临床分期、淋巴结转移和远处转移相关(P<0.05);(3)HPSE阳性表达患者中位生存时间为11.0个月,阴性表达患者中位生存时间为70.0个月(P<0.05)。结论:测定胃癌组织中HPSE的表达,有助于临床胃癌患者的诊断以及预后判断。Objective ： To investigate the expression of heparanase （HPSE） in gastric cancer and their relationship with clinical pathologic character and with survival, and to discuss the feasibility of HPSE as prognostic markers. Methods ：The expression of HPSE in gastric cancer were studied by inununohistochemical technique with their relation to follow-up data, analyzed their detect outcome with clinical pathologic character. Results： （ 1 ）The 59 of 103 gastric cancer specimens （57.3%） were positive for HPSE. （2）Its ex- pression was not coIrelated with sex, age, size and site of tumor and degree of differentiation （ P 〉 0. 05 ）. The positive expression rates of HPSE were related to depth of invasion,clinical stage,lymph node and distant metastasis of gastric cancer patients （P 〈 0. 05 ）. （3） Median survival period of patients with HPSE positive was 11.0 months, median survival period of patients with HPSE negative was 70. 0 months, respectively. There was significant difference between median survival period of patients with HPSE positive and negative （ P 〈 0. 05 ）. Conclusion ： Detecting the expression of HPSE in gastric cancer tissues can shed light on the judgement the prognosis and guide treatment for patients.
