杏丁注射液治疗慢性肾炎及对患者尿内皮素的影响和意义

摘　　要：目的探讨杏丁注射液在治疗慢性肾炎中的作用及机制。方法将50例患者分为两组:常规治疗组25例(简称常规组)和常规+杏丁治疗组25例(简称杏丁组),观察治疗前后肾功能、尿蛋白定量及尿ET-1水平的变化。结果两组患者治疗前血清Scr、24h尿蛋白定量、尿ET-1水平差异无显著性(P>0.05),但均高于对照组(P<0.01),杏丁注射液治疗后尿ET-1水平明显减少(P<0.01),且与常规组治疗后比较也有显著下降(P<0.05),但仍高于正常人。而常规组治疗前后各项指标均无明显变化(P>0.05)。相关性分析:杏丁组治疗后尿ET-1水平与尿蛋白定量呈正相关(r=0.5920,P<0.01)。结论CGN患者尿ET-1明显增多,杏丁注射液可能通过抑制CGN患者体内尤其肾脏产生的ET-1致尿液排泄ET-1减少,从而对CGN患者肾脏功能起到一定的改善和保护作用。Objective To study the effect and clinical significance of xingding injection on urinary endothelin-1 （ET-1） excretion in treating primary chronic glomerulonephritis （CGN） diseases. Methotis Patient with CGN diseases were randomly divided into two groups which were the usual medicine treatment group （UT group, n= 25 ） and medicine and xinding injection treatment group （XT group, n= 25）. The UT group was treated with multi-glycosides Tripterygium wilfordii tablets, persatin, baoshenkang and antihypertensive. The XT group was treated with usual medicine and xinding injection and so on. The urinary ET-1 of CGN patients were higher than those in normal group before treatment （P〈0. 01）. The urinary ET-1 excretion was detected before and after treatment, and also control group. ResultsThe urinary levels of ET-1 of CGN patients were higher than those in normal group before treatment （P〈0. 01）. The changes of urinary levels of XT group before and after treatment were significant （P〈0. 01）. The improvement of urinary ET-1 in the XT group after treatment was better than that in the UT group （P〈0. 05）. The urinary levels of ET-1 of XT group after treatment were significantly and positively correlated with the urinary protein （r=0. 5920,P〈0. 01）. Conclusion Xinding injection may provide significant modification and protection for kidney in patients with CGN by inhibition of the production of ET-1, especially in kidney of patients with CGN. Therefore, it leads to decrease ET-1 excretion in the urinary. Xinding injection may be an effective drug for the clinical treatment of patients with CGN.
