早期运动康复干预对脊髓损伤患者功能恢复的影响

摘　　要：目的:探讨运动康复干预对脊髓损伤患者运动功能改善、日常生活能力和心理状况的影响。方法:采用随机数字法把70例脊髓损伤患者分为治疗组40例和对照组30例;治疗组从伤后4周后开始系统运动康复干预3个月,对照组只在早期进行康复指导未行系统运动康复干预。两组分别于伤后4周和伤后满4个月(系统运动干预前后)进行Rivermead运动指数,Barthel日常生活能力指数,SAS焦虑自评量表的综合功能评定。结果:治疗后Barthel指数为69.37±10.65,与干预前32.28±11.43比较,有显著提高(P<0.01);治疗后Rivermead指数5.03±2.96,与干预前3.28±2.47比较,有显著提高(P<0.01);治疗后SAS分值为41.76±8.68,与干预前48.17±6.36比较,有显著提高(P<0.01)。结论:运动康复干预对脊髓损伤患者的功能恢复有重要的意义,能有效改善患者运动功能、日常生活独立能力和心理状态。Objective： To investigate the effect of athletic intervention therapy in early stage of spinal cord injury patients on the recovery of patient ＇s athletic function, activity of daily living and psychological status. Methods： 70 cases of spinal cord injury patients were randomized classed into therapy group （n = 40） and control group （n = 30 ）. Athletic intervention therapy began 4 week ＇after injured in therapy group and 4 months in control group. Rivermead index and Barthel index and Self-rating Anxiety Scale were used to evaluate the rehabilitation of both groups. Results： The scores of three kinds of assessments all change significantly in therapy group compared with those in control group after the rehabilitation therapy. The amplitude of improvement in the therapy group differs significantly with that in the control group. Compared with before and after therapy in control group, Barthel index was increased from 32.28 ±1 1.43 to 69.37 ± 10.65 （ P 〈 0.01 ）, Rivermead index was increased from3.28 ± 2.47 to 8.31 ±3.45 （ P 〈 0. 01 ） ; SAS score was decreased from48. 17 ± 6.36 to 41. 76± 8.68 （ P 〈 0. 01 ） Conclusion： Athletic rehabilitation treatment improves the functional recovery of patients with spinal cord injuries.
