多层螺旋CT血管造影在下肢血栓闭塞性脉管炎中的诊断价值

摘　　要：目的探讨多层螺旋CT血管成像(multi-slice computed tomographic angiography,MSCTA)在下肢血栓闭塞性脉管炎(thromboangiitis obliterans,TAO)中的诊断价值。资料与方法对临床可疑下肢TAO的43例患者行MSCTA检查,扫描范围自膈下至踝关节水平,扫描原始图像经容积再现(VR)、最大密度投影(MIP)重组,显示该病变的特征性CT表现。9例同时行DSA检查,25例行手术治疗。结果43例MSCTA表现为单侧或双侧下肢动脉狭窄、闭塞,周围见侧支动脉,其中15例显示小腿动脉内血栓,27例显示TAO所特有的螺旋状动脉。结论MSCTA可以清晰显示TAO的下肢动脉病变,通过后处理技术能显示该病的特征性CT表现,为临床诊断和治疗提供良好的依据。Objective To assess the diagnostic value of muhi-slice computed tomographic angiography （MSCTA） in thromboangiitis obliterans （TAO, Buerger＇s disease）. Materials and Methods MSCTA was performed on 43 patients with clinical suspicious of TAO. The scanning started at the level beneath the diaphragm, and ended at ankle. CTA image was obtained from reconstructed original imaging with volume rendering and maximum intensity projection. 9 patients also had digital substraction angiography. 25 of the 43 patients received surgical interventions. Results Arterial stenosis or occlusion with collateral vessels in unilateral or bilateral lower extremities were shown in MSCTA, arterial thrombosis in calf was seen in 15 cases, and the characteristic screw - like appearance of Buerger＇s disease was seen in 27 cases. Conclusion Arterial lesions of lower extremity in patients with Buerger＇s disease could be clearly identified by MSCTA, MSCTA is a valuable method in diagnosis of Buerger＇s disease.
