摘 要:目的探讨血必净注射液对严重烧伤患者重要脏器功能的作用。方法将同时期严重程度相当的重度以上烧伤住院患者58例随机分为治疗组(n=29)和对照组(n=29)。对照组按烧伤常规治疗,治疗组在常规治疗的基础上加用血必净注射液。特重度烧伤每次150mL,重度烧伤每次100mL,每日2次,连续使用7d。检测两组患者治疗前后的体温、心率、呼吸、血常规、肝功能、肾功能、心肌酶、C反应蛋白(CRP)及血浆内毒素(LPS)等变化。结果严重烧伤患者早期,重要脏器功能通常都会出现较明显的异常变化,血浆LPS水平在伤后3d(治疗后3d)即会明显升高。在常规治疗的基础上应用血必净注射液后3d,可明显降低血浆LPS水平,CRP亦明显改善。用药后7d,患者生命体征和重要脏器功能指标均有一定程度的改善,未见明显不良反应。结论血必净注射液能有效保护严重烧伤患者的脏器功能,适用于严重烧伤的早期治疗。OBJECTIVE To investigate the protective effects of Xuebijing injection on important organ function in severe burn. METHODS Fifty eight cases with comparative severity in burn injury were randomly divided into Xuebijng injection treatment group (n= 29) and control group (n= 29). Patients in control group received routine therapy, while those in treatment group received intravenous dripping of Xuebijing injection in twice a day for 7 days. The dosage of Xuebijing injection was 150mL in severe bums and 100mL in moderate burns. The clinical findings were assessed and variables indicating functions of internal organs, including temperature (T), heart rate (HR) ,respiration ( R), blood routine test, liver, kidney, myocardial enzymes, c-reactive protein (CRP) and lipopolysaccharide (LPS) were detected. RESULTS In the early days, functions of important organ in patients with severe burn have abnormal changes apparently, and the level of LPS in plasma would be elevated obviously after 3 days of injury (3 days of treatment). 3 days after using Xuebijing injection on the base of routine therapy, the level of LPS in plasma was dropped markedly, and CRP improved significantly. Physical sign and important organ functions index signs of patients were all improved partly without notable bad reactions after 7 days using Xuebijing injection on the base of routine therapy. CONCLUSION Xuebijing injection has protective effects on multiple organs in severe burn injury and apply to the early treatment of severe burn.
