不同手术方式治疗声带息肉的疗效比较

摘　　要：目的比较显微支撑喉镜与纤维喉镜下摘除声带息肉的疗效,探讨不同声带息肉手术方式的选择。方法回顾性分析我院1998年1月至2008年11月采用这两种方法治疗的声带息肉患者共590例,其中显微支撑喉镜组404例,纤维喉镜组186例。术后随访3月,对比分析两组的疗效,并对比复杂型及双侧声带息肉患者的疗效。结果显微支撑喉镜组和纤维喉镜组的疗效无明显差别(P=0.067),复杂型声带息肉患者在显微支撑喉镜下手术的疗效优于纤维喉镜下手术(P=0.001),双侧声带息肉患者显微支撑喉镜下手术与纤维喉镜下手术疗效无明显差别(P=0.096)。结论复杂型声带息肉患者采用显微支撑喉镜下手术的疗效优于纤维喉镜下手术,而对于双侧息肉患者的疗效无明显差别。Objective To compare the therapeutic effects of suspension laryngoscopes in microlaryngoscopic surgery and fibrolaryngoscopic surgeries on treating vocal cord polyps, and to discuss the choice of different laryngoscopic surgeries. Methods Clinical data of 590 patients with vocal cord polyps from Jan. 1998 to Nov. 2008 were analyzed retrospectively in which 404 cases were treated under suspension laryngoscope by microlaryngoscopic surgery （suspension laryngoscope group） and 186 cases were treated by fibrolaryngoscopic surgery （fibrolaryngoscope group ）. The duration of fellow-up was 3 months in all patients. The clinical effects of two kinds of surgeries were observed and the effects on complicated and bilateral types of vocal cord polyps were compared. Results The clinical effects in suspension laryngoscope group showed no difference as compared with those in fibrolaryngoscope group （ P = 0. 067 ）. For patients with complicated vocal cord polyps, the effect of suspension laryngoscopic surgery was significantly better result as compared with that in the fibrolaryngoscopic surgery group （ P = 0. 001 ）, while for bilateral type patients there was no difference between two surgeries （ P = 0. 096 ）. Conclusion The operation with suspension laryngoscopes in microlaryngoscopic surgery is the first choice for the treatment of patients with complicated vocal cord polyps, however, the clinical effects are similar by two types of surgeries for patients with bilateral vocal cord polyps.
