妊娠期急性阑尾炎47例临床分析及心理护理

摘　　要：目的:了解妊娠期急性阑尾炎的发病特点,诊断中的特殊性、治疗及心理反应,提高对本病的认识和重视,降低由于误诊或延误手术导致并发症引起母婴死亡率以及由于心理应激对病情的影响。方法:对2003年1月-2008年12月收治的47例妊娠期急性阑尾炎病例进行回顾性分析。结果:本组47例的治愈出院,其中早孕2例,中孕27例,晚孕18例。合并急性坏疽性穿孔16例,其中中孕10例,晚孕6例,误诊1例。通过手术治疗切除阑尾43例,保守治疗4例(其中2例复发后行手术治疗),流产1例,早产1例。经心理干预后病人解除焦虑和恐惧,积极配合治疗。结论:妊娠期急性阑尾炎多数病例发生在中晚孕期;及时诊断和手术治疗是处理本病的关键;积极的心理干预对病人的恢复起一定作用。Objective： To understand the incidence of acute appendicitis during pregnancy characteristics, specificity of diagnosis. Treatment and psychological reactions, enhance awareness of the disease and the importance to reduce as a result of misdiagnosis or delay in surgery led to complications caused by materhal and child mortality as well as psychological should be shock of the impact of the disease. Ways of January 2003 - 2008 was admitted in December of 47 cases of acute appendicitis in pregnancy eases were retrospectively analyzed. Results：The 47 cases of patients discharged, of which two cases of early pregnancy, in pregnancy 27 cases, 18 cases of late pregnancy. With acute gangrenous perforation of 16 cases ,of which 10 cases of pregnancy, six cases of late pregnancy, one cases of misdiagnosis. Appendix removed through the surgical treatment of 43 cases,four cases of conservative treatment （ including two eases of recurrence later surgery） ,1 cases of miscarriage,premature Example 1. By psychological intervention in patients after the lifting of anxiety and fear, actively cooperate with the treatment. Conclusion： Most cases of acute appendicitis in pregnancy occur in late pregnancy;timely diagnosis and surgical treatment are the key to deal with this disease;positive psychological intervention for patients to resume a role to play.
