中药穴位贴敷治疗原发性痛经实证患者31例临床观察

摘　　要：目的观察中药穴位贴敷治疗原发性痛经实证患者的临床疗效及其对前列腺素(PGs)的影响。方法将原发性痛经实证患者随机分为治疗组(33例)和对照组(31例),治疗组行中药穴位贴敷,对照组口服田七痛经胶囊。治疗前后分别观察两组患者痛经的疼痛积分以及伴随症状,并测定外周血前列腺素F2α(PGF2α)与前列腺素E2(PGE2)含量。结果两组患者治疗后疼痛积分均较治疗前明显降低(P<0.01);治疗组疗效、疼痛积分及伴随症状改善程度均较对照组显著(P<0.05或P<0.01)。治疗后两组患者外周血PGE2含量增高,PGF2α及PGF2α/PGE2下降,与治疗前比较差异有统计学意义(P<0.05或P<0.01);治疗组PGE2升高程度和PGF2α/PGE2的下降程度大于对照组(P<0.01或P<0.05)。结论穴位贴敷治疗原发性痛经实证有较好的临床疗效,并对原发性痛经患者前列腺素的合成有良性调节作用。Objective To observe the effect of herbal plaster at acupoints for the excess syndrome of primary dysmenorrhea and its influence on prostaglandins. Methods The patients with of primary dysmenorrhea excess syndrome were randomized into treatment group （33 cases） and control group （31 cases）. The former was treated by herbal plaster at acupoints while the latter treated by orally taking Tianqi TongUing Capsule （Radix Notoginseng for Capsule Dysmenorrhea）. Before and after the treatment, the pain scores and the accompanied symptoms were observed; and PGF2α and PGE2 of peripheral blood and renal and hepatic functions were detected. Results After treatment, the pain scores of both groups were significantly decreased （P〈0. 01）, the therapeutic effect, pain score, and the relief of accompanied symptoms in the treatment group were significantly better than those in the control group （P〈0. 05 or P 〈0.01）. In both groups, the content of PGE2α of peripheral blood was increased and the content of PGF2α and the PGF2α/PGE2 were decreased, compared with those before treatment, the difference was significant （P〈0. 01 or P〈0. 05）. The PGE2 increase and PGF2α/PGE2 decrease of the treatment group were more than those of the control group （P〈0. 01 or P〈0. 05）. Conclusion Herbal plaster at acupoints is effective for the excess syndrome of primary dysmenorrhea, and it is able to regulate the synthesis of prostaglandin.
