载脂蛋白E基因多态性与子痫前期的相关性

摘　　要：目的探讨载脂蛋白E(apolipoprotein E,ApoE)基因多态性与子痫前期发生的关系。方法应用聚合酶链反应-限制性片段长度多态性分析(PCR-RFLP)技术对2007年1月至12月广东省妇幼保健院96例子痫前期患者和98例正常孕妇进行不同ApoE基因型分析,并观察两组血脂情况。结果(1)子痫前期患者血清总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)显著升高,高密度脂蛋白胆固醇(HDL-C)显著下降(P<0.05)。(2)在研究总人群中,ApoE等位基因频率分布分别为ε2(5.7%),ε3(85.8%),ε4(8.5%)。在不同的ApoE基因型组,血脂水平不同,并且差异有统计学意义(P<0.01)。TC、TG和LDL-C水平均按E3/4+E4/4组>E3/3组>E2/2+E2/3组的顺序降低,HDL-C水平按上述顺序升高。(3)子痫前期组ε3等位基因频率79.7%,明显低于对照组91.8%(P<0.05),ε4等位基因频率13.0%,明显高于对照组的4.1%,差异有统计学意义(P<0.01)。结论ApoEε4等位基因可能是子痫前期的遗传危险因子。ApoEε4等位基因与高脂血症相关。Objective To evaluate the relationship between ApoE polymorpbism and the blood lipid level, and the rela- tionship between ApoE polymorphism and pre-eclampsia. Methods ApoE polymorphism was genotyped by the polymer- ase chain reaction-restriction fragment length polymorphism （PCR-RFLP） method in 96 pre-eclampsia cases and 98 healthy pregnant controls from January to December in 2007. The serum lipid level of both groups was also determined. Results The serum TG, TC and LDL-C in PE patients were significantly higher than in healthy pregnant women （ P 〈 0.05 ）. The serum HDL-C was lower in PE patients than in healthy pregnant women （ P 〈 0. 05 ）. The ApoE allele fre- quency was s2（5. 7% ）,s3（85.8% ）,e4（8. ：5% ） respectively in our study. The levels of serum lipid were different significantly in different genotypes of ApoE （P 〈0. 01 ）. The levels of TC,TG and LDL-C decreased in the order of E3/ 4 ＋ FA/4 〉 E3/3 〉 E2/2 ＋ E2/3, and the level of HDL-C was just the reverse（ P 〈 0. 01 ）. The frequency of ε3 was lower in the cases than in the controls （79. 7% vs 91.8% , P 〈0. O1 ） , and the frequency of ε4 was higher in the cases than in the controls （ P 〈 0.01 ）. Condnsion ApoE ε4 allele may be an genetic risk factor of pre-eclampsia, and there is a sig- nificant association between ApoE ε4 allele and hyperlipemia.
