摘 要:目的:探讨应用大剂量重组人促红细胞生成素(rHuEPO)治疗肾性贫血的效果及机制。方法:对38例处于慢性肾脏病(CKD)3~5期的贫血患者给予佳林豪1.2万IU皮下注射,每周1次,连续12周,同时补充铁剂、叶酸和维生素B12;骨髓穿刺进行铁染色和显微图像分析,比较治疗前后血清铁蛋白(SF)、血清铁(SI)及铁粒幼细胞计数,对总红系所占百分数(E(与SI、SF、红细胞压积(He0行相关分析,治疗前后取血检测红细胞压积(HCT)、RBC、血红蛋白(Hb)。结果:治疗过程中大部分患者在EPO注射12周后HCT、RBC、Hb均比用药前有显著增高,骨髓红系增生较治疗前明显活跃,各类幼红细胞比率及E(总红系所占百分数)均显著升高,其中以晚幼红增生最为明显,SI、SF及铁粒幼细胞计数均明显降低,治疗前后骨髓AE%与血ARBC%、AHCT%呈高度正相关,AE%与ASF%、铁粒幼细胞计数变化率呈中度负相关,AE%与ASI%两者呈低度负相关。结论:大剂量重组人促红细胞生成素治疗肾性贫血效果显著。Objective: To study the effect of high dose of rHuEPO for renal anemian on bone marrow and iron metabolism. Methods: 38 patients with renal anemia received 120001U per weeks of Jialinhao hypodermic injection subcutaneous for 12weeks, meanwhile they took folio acids, vitamins B12 and chalybeates. Before and after the cure, take a sample of blood to test HCT,RBC,Hb Bone marrow puncture and Bone marrow micrograph anlysis was performed,serum ferritin and serum iron before and after the therapy and their corre- lation were analyzed. Results: After the therapy, RBC, Hb, HCTincreased significantly, After the therapy marrow iron stain, serum fer- ritin and serum iron decreased significantly, correlation AE% strongly correlated with ARBC%, AHCT% (P〈0.001). AE% negatively cor- related with the variety ratio of sideroblast counts, ASI% and ASF% (P〈0.05). But ASI% and ASF% had no correlation with ARBC% and AHCT%. Conclusions: High dose of recombinant human erythropoietin treatment of renal anemia results significantly.
