初诊2型糖尿病患者短期胰岛素强化治疗的临床研究

摘　　要：目的观察短期胰岛素强化治疗初诊的2型糖尿病患者的疗效。方法将新诊断的2型糖尿病患者138例随机分为2组,观察组78例予胰岛素强化治疗,对照组60例给予口服降糖药物治疗,比较2组治疗前后空腹血糖(FPG)、餐后血糖(2hPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、C-肽(C-P)、胰岛素抵抗指数(HOMA-IR)和胰岛素分泌指数(HOMA-β)。结果治疗后2组的FPG、2hPG、HbA1c、FINS、C-P、HOMA-IR、HOMA-β各项指标均较治疗前有所改善(P<0.05);且治疗组PFG、2hPG、HOMA-IR下降指数较对照组明显,FINS、C-P、HOMA-β升高指数较对照组明显,2组差异有统计学意义(P<0.05)。结论初诊的2型糖尿病患者予短期胰岛素强化治疗有利于改善血糖和胰岛功能。Objective To investigate the therapeutic effect of short-term intensive insulin treatment on newly diagnosed type 2 diabetes mellitus patients. Methods For the newly diagnosed type 2 diabetes patients, Oral Antidiabetics and short-term intensive insulin treatment separately. Therapeutic effects were compared between two groups the therapeutic effect, fasting plasma glucose （ FPG）, postprandial 2 hours plasma glucose （2hPG）, hemoglobin A1 c （ HbA1 c）, fasting insulin （FINS）, insulin resistance index（HOMA-IR） ,and insulin resistance index （HOMA-[$） were observed before and after treatment. Re- suits After treatment,the level of FPG,2hPG,HbAle,FINS,C-P,HOMA-IR,HOMA-~were all improved（P 〈0.05） ,and there was significant difference between two groups（P 〈 O. 05）. Conclusion Short-term intensive insulin treatment on newly diagnosed type 2 is helpful to controlling blood glucose and islet function.
