慢性阻塞性肺疾病肺部振动反应成像的特点

摘　　要：目的探讨慢性阻塞性肺疾病(COPD)肺部振动反应成像(VRI)的特点。方法63例门诊COPD患者行支气管舒张试验前后肺功能和VRI检查,66名健康志愿者行肺功能和VRI检查。记录VRI结果包括右肺肺部定量数据(QLD)值、啰音数和图像评分,对结果进行分析比较。结果COPD组支气管舒张试验后右肺QLD值为(51.0±12.1)%,啰音均数为2.7,图像评分为4.2±1.9;对照组右肺QLD值为(45.1±5.9)%,啰音均数为0.1,图像平均分为0.9;两组间差异均有统计学意义(P<0.01)。COPD组支气管舒张试验前后VRI结果差异无统计学意义(P>0.05)。支气管舒张试验后VRI图像评分与FEV1%呈线性相关关系(r=0.31,P=0.01),回归方程为y=63.85-2.95x(y:100×FEV1%,x:VRI图像评分)。结论COPD患者VRI结果与正常人存在显著差异,且支气管舒张试验前后无明显差异,可协助COPD的诊断。支气管舒张试验后VRI图像评分对估计气流受限程度有一定价值。Objective To explore the characteristics of vibration response imaging （VRI） in chronic obstructive pulmonary disease （ COPD ）. Methods Sixty-three COPD patients received lung function test （LFT） and VRI examination before and after inhalation of bronchodilator. As the control group, 66 healthy volunteers received LFT and VRI examinations. We defined VRI results as quantitative lung data （ QLD ） , crack counts, and image grade. Results The results of control group were： QLD of right lung （45.1 ± 5.9% ） , mean crack counts 0.1, and mean image grade 0. 9. After inhalation of bronchodilator, the VRI resuits of COPD group were ： QLD of right lung （51.0 ± 12. 1 ） % , mean crack counts 2.7, and image grade 4. 2 ± 1.9. The results between control group and COPD group were significantly different （ P 〈 0.01 ）. The VRI resuits were similar before and after inhalation of bronchodilator in COPD group （ P 〉 0.05 ）. After inhalation of bronchodilator, there was a linear relationship between VRI image grade and FEV1% （r =0.31, P =0.01 ）. The model of the linear regression was： y =63.85 -2.95x （y： 100 x FEV1% , x： VRI image grade）. Conclusions The VRI results differ between COPD patients and healthy people. The VRI results are stable after inhalation of bronchodilator in COPD patients, which may be helpful in the diagnosis of COPD. The VRI image grade after inhalation of bronchodilator is useful to assess the degree of obstruction.
