心理干预对鼻咽癌患者情绪障碍和生活质量的影响

摘　　要：目的探讨心理干预对鼻咽癌患者情绪障碍和生活质量的影响。方法收集确诊为鼻咽癌半年以内，未转移或复发，未合并其他慢性疾病的患者48例。采用SCL-90、SAS、SDS和EORTC QLQ—C30对其进行精神心理状态和生活质量的追踪评估，并根据初期的评估后果进行有针时性的心理干预，并对干预结果进行分析。结果鼻咽癌确诊初期患者的心理健康状况较差，焦虑、抑郁情况明显，生活质量明显下降；初期各评定结果分别为SCL-90（197．75±1．82）分、SAS（44．90±7．98）分、SDS（44．49±7．52）分和EORTCQLQ-C30（60．08±4．48）分。干预半年后，患者无论在心理状态和生活质量上均较半年前有显著改善，各量表评分分别为SCL-90（157．26±13．66）分、SAS（41．94±4．86）分、SDS（40．53±4．61）分和EORTCQLQ—C30（67．96±6．68）分；前后分值比较差异均有统计学意义（p〈0．01或p〈0．05）。结论对确诊为鼻咽癌的初期患者实施有效的心理干预，可以减轻患者的负性心理状态，提高生活质量。Objective： Discussion of psychological intervention on emotional disorders in patients with nasopharyngeal carcinoma and quality of life.Methods： Collection of nasopharyngeal carcinoma diagnosed with in six months, no transfer or relapse, not with other chronic diseases of 48 cases of patients using SCL-90,SDS, SAS and the EORTC, QLQ-C30 on their psychological state and quality of life follow-up assessment, and in accordance with the initial assessment of the consequences of targeted psychological intervention ,and intervention were analyzed.Results：The early diagnosis of nasopharyngeal carcinoma patients with poor mental health status,anxiety and depression is evident, the quality of life decreased significantly ;the early results of the assessment of SCL-90（197.75±1.82）min,SAS （44.90±7.98）points ,SDS（44.49±7.52）points and EORTC QLQ-C30 （60.08±4.48）min.Six months after the intervention,patients in both psychological state and quality of life than the half a year ago has significantly improved,the scale score of SCL-90 （157.26±13.66）min,SAS （41.94±4.86）points,SDS （40.53±4.61） points and EORTC QLQ-C30 （67.96±6.68）rain;before and after the scores were statistically significant difference between （p〈0.01 or p〈0.05）.Conclusion：To implement efficient psychological intervention for the early diagnosis of nasopharyngeal cancer patients can relieve the patient＇s negative mental state,improve the quality of life.
