摘  要:目的探讨血管紧张素转换酶(ACE)基因I/D多态性与高血压病血瘀证(EH-BSS)首次心脑血管事件的关系。方法收集EH-BSS患者532例(EH-BSS组),高血压病非血瘀证298例(EH-non-BSS组),血压正常对照者512例(Control组),抽取外周静脉血提取DNA,采用聚合酶链反应(PCR)检测ACE基因I/D多态性类型。结果卡方分割显示EH-BSS组的DD频率(18.05%)高于EH-non-BSS组(11.74%)、Control组(10.74%),有统计学意义(χ2=12.931,P<0.001)。多因素Logistic回归分析,调整混杂危险因素后,ACE-DD型患EH-BSS的OR值为1.877(95%CI:1.140~3.088,P=0.013)。Kaplan-Meier回顾性分析,携带ACE-DD型患者在高血压病史前15年发生血瘀证首次心脑血管事件风险明显高于携带II/ID型患者(χ2=10.056,P=0.002)。结论携带ACE-DD基因型是患EH-BSS独立危险因素,尤其在EH病史前15年发生BSS心脑血管事件风险明显增高。Objective To discuss the relation between I/D polymorphisms of angiotension-concerting enzyme (ACE) gene and the first cardiac vascular accidence ( CVA ) in hypertension of blood stagnation syndrome. Methods 532 cases of hypertension of blood stagnation syndrome( BSS group) ,298 cases of hypertension without blood stagnation syndrome ( non - BSS group) and 512 cases of normal blood pressure (control group)were collected. DNA was extracted from the peripheral vein and I/D polymorphisms of ACE gene was determined by polymerase chain reaction(PCR). ReSults The partition of chi square indicated that DD frequency (18.05%)in BSS group was higher than that in non -BSS group (11.74%)and control group (10.74%) ,presenting statistical significance(X^2=12.931 ,P 〈0.001). After muhifactor logistic - regression a- nalysis and adjustment of mixed risk factors, OR value of ACE - DD genotypes was 1.877(95% CI : 1.140-3.088 ,P = 0.013). Kaplan -Meier retrospective analysis indicated that the risk of the first CVA with blood stasis syndrome of ACE - DD carrier was higher apparently compared with that of Ⅱ/ID carrier(X^2=10.056, P=0.002) in earlier 15 years of hypertension history. Conclusion ACE - DD genotype carrier Was the independent risk factor of hypertension with blood stasis syndrome, especially, the risk of CVA of blood stasis syndrome increases apparently in the earlier 15 years of hypertension history.
