烤瓷全冠直接修复活髓隐裂牙疗效观察

摘　　要：目的:探讨金属烤瓷冠直接修复活髓隐裂牙的疗效,以指导口腔临床对活髓隐裂牙的处理。方法:87例(132颗牙)隐裂牙患者按临床症状分为轻微咬合痛组(24例,31颗牙,第1组)、一过性冷热刺激痛组(33例,56颗牙,第2组)、延续性冷热刺激痛组(30例,45颗牙,第3组)3组,直接进行全冠修复,观察修复1周、半年和1年后急性牙髓炎的发生率。结果:全冠修复1周,第1组治愈率为100%;第2组治愈率为91.0%,改善率为100.0%;第3组治愈率为69.6%,改善率为87.0%,失败率为13.0%。全冠修复半年,第1、2组治愈率为100.0%,第3组治愈率为80.0%,失败率为20.0%。全冠修复1年,第1、2组无变化,第3组治愈率为75.6%,失败率为24.4%。结论:对于轻微咬合痛和一过性冷热刺激痛的患牙,可以直接行烤瓷冠修复,对于有延续性冷热刺激痛的患牙,建议先进行根管治疗再行全冠修复。Objective： To investigate the effect of porcelain crown on direct repair of cracked vital pulp teeth and to guide clinical treatment on cracked vital pulp teeth. Methods： One hundred thirty-two cracked teeth of 87 patients were divided into 3 groups according to clinical symptoms ： group A （24 cases, 31 teeth）, mild pain at occlusion; group B： transient pain to cold stimulation （33 cases, 56 teeth） ; group C： continuous pain to cold stimulation （30 cases, 45 teeth） ; all teeth were undergoing whole crown repair, the incidence of acute pulpitis was observed after repair in 1 week, 6 months and 1 year. Results： One week after the operation, the cure rate in group A,B and C was 100%, 91.0% and 69.6% respectively, the improvement rate in group A,B was 100% and 87.0% respectively, the failure rate in group C was 13.0 %. Six months after the operation, the cure rate in group A,B and C was 100%, 100% and 80.0 % respectively, the failure rate in group C was 20.0 %. One year after the operation, the cure rate in group A,B and C was 100%, 100% and 75.6 % respectively, the failure rate in group C was 24.4 %. Conclusion ： Direct repair with porcelain crown indicates cracked vital pulp teeth with mild pain at occlusion and transient pain to cold stimulation ; the whole crown repair of cracked vital pulp teeth with continuous pain to cold stimulation is proposed to follow the root canal treatment.
