摘  要:目的探讨依托咪酯脂肪乳剂(宜妥利)在无痛人工流产术中应用的可行性及剂量。方法选择60例择期行无痛人工流产术的妇女随机分为A、B两组,A组静脉推注宜妥利0.4mg/kg+芬太尼1μg/kg,B组给予宜妥利0.3mg/kg+芬太尼1μg/kg,观察不同时间点的呼吸循环变化及不良反应的发生。结果宜妥利用在无痛人工流产术中无注射痛,患者呼吸循环稳定,各时点血压、心率、呼吸比较无显著性差异。B组麻醉时间、麻醉诱导时间、麻醉恢复时间均短于A组(P<0.05)。恶心呕吐发生情况两组无显著性差异。重度肌阵挛的发生率B组明显少于A组(P<0.05)。结论宜妥利0.3mg/kg+芬太尼1μg/kg用在无痛人流术中患者呼吸循环稳定,但恶心呕吐、肌阵挛的发生率较高,仍是值得关注的问题。Objective To investigate the feasibility and dose of Etomidate -Lipuro in anedynia artificial abortion. Methods The study involved 60 anodynia artificial abortion patients who were placed randomly in group A and group B. The patients in group A accepted intravenous injection of Etomidate -Lipuro (0.4mg/kg) and fentanyl (1 μg/kg), and the patients in group B accepted intravenous injection of Etomidate - Lipuro (0.3mg/kg) and fentanyl (1μg/kg). Then the changes of the respiration and circulation at different times and the side reaction were observed. Results The patients accepted Etomidate - Lipuro have no injection pain in anodynia artificial abortion. The respiration and circulation of the patients are stable. At different times there is no significant difference in blood pressure, heart rate and respiration. The anesthesia induction time, the anesthesia duration, and the anesthesia recovery time in group B are shorter significantly than group A ( P 〈 0.05 ). There is no significant difference in nausea and vomiting, but the number of the patients who had severe myoclonus is less significantly in group B than group A (P 〈 0.05). Conclusions The respiration and circulation of the anodynia artificial abortion patients are stable after intravenous injection of Etomidate - Lipuro (0.3mg/kg) and fentanyl (1 μg/kg), however, the incidence of nausea, vomiting and myoclonus are high.
