超声心动图评价Gore-Tex人工腱索植入二尖瓣成形术中期效果

摘　　要：目的应用经胸超声心动图(TTE)评价二尖瓣成形术中植入Gore-Tex人工腱索治疗二尖瓣腱索部分断裂的效果和应用前景。方法12例二尖瓣部分腱索断裂患者,其中男性7例,女性5例,年龄22～73岁,平均年龄55.58岁。前叶腱索断裂2例,后叶腱索断裂10例。行Gore-Tex人工腱索植入二尖瓣成形术,在术前及术后进行TTE检查,评价反流程度,测量左心室舒张末期内径(LVDd)、左心房内径(LAID)、肺动脉收缩压(PASP)和左心室射血分数(LVEF)。结果TTE显示植入Gore-Tex人工腱索后,二尖瓣反流较术前有明显减轻,术后LVDd、LAID及PASP与术前比较明显减小(P<0.05),LVEF术后与术前比较差异无统计学意义(P>0.05)。结论Gore-Tex人工腱索植入二尖瓣成形术患者中期的TTE随访显示Gore-Tex人工腱索植入二尖瓣成形术有较好的疗效,但更远期疗效有待进一步研究。Objective To evaluate the effects,prospects and applications of mitral valvuloplasty with Gore-Tex suture as an artificial chordae tendineae in patients with partial rupture chordae tendineae of mitral leaflet diagnosed by transthoracic echocardiography（TTE）. Methods A total of 12 patients（male 7, female 5, aged 22 - 73 years ,mean age 55.58 years） with cardiac disfunetion caused by partial rupture chordae tendineae of mitral valve leaflet, which received mitral valvuloplasty with Gore-Tex suture as an artificial chordae tendineae. The degree of mitral regurgitation, left ventricular dimension in end-diastole（LVDd） and left atrium internal diameter （LAID） as well as pulmonary artery systolic pressure （PASP） were detected before and after the operation with TIE. Results After Gore-Tex artificial chordae tendineae implantation in mitral valvuloplasty, mitral regurgitation decreased remarkably. LVDd, LAID and PASP decreased significantly compared with before operation （P 〈 0.05）. There was no statistical difference in LVEF after operation（P 〉 0.05）. Conclusion The mid-term follow-up with TIE of Gore-Tex artificial chordae tendineae implantation in mitral valvuloplasty indicates that operation have satisfactory effect, but more long-term results have not studied.
