自然周期未成熟卵体外培养治疗多囊卵巢综合征性不孕症的临床研究

摘　　要：目的研究在自然周期获取未成熟卵母细胞体外培养成熟(IVM)治疗多囊卵巢综合征(PCOS)性不孕症的临床疗效。方法57例行体外受精-胚胎移植助孕的PCOS患者,共62个周期,获取434个未成熟卵母细胞进行体外培养,成熟后行单精子卵胞浆内注射,受精后取优质胚胎移植宫腔,统计获卵数、受精率、卵裂率和妊娠率。结果62个周期共培养出成熟卵子293个,成熟率为67.5%;271个卵子受精,受精率92.5%,卵裂率81.2%,其中52个周期进行胚胎移植,周期取消率为16.1%;17例患者妊娠,移植周期妊娠率32.7%。结论IVM是治疗多囊卵巢综合征性不孕症的有效方法。Objective To study the effect of human oocytes from natural cycle in vitro maturation applied for treatment of infertility in women with polycystic ovary syndrome. Methods Totally 434 immature oocytes were retrieved from 57 PCOS women and cultured in vitro. The mature oocytes were inseminated by intracytoplasmic sperm injection（ 1CSI）, then embryo transplantations were performed. Number of retrieved oocytes, fertilization rate, cleavage rate and pregnancy rate were recorded and analyzed by statistical software. Results Totally 293 mature oocytes were obtained. The rates of maturity, fertilization and cleavage were 67.5%, 92.5% and 81.2% respectively. Embryo transplantation was performed in 52 of them, and cancellation rate was 16.1%. 17 women were pregnant and pregnant rate was 32.7%. Conclusion The oocytes from natural cycle in vitro maturation can be effectively applied for treatment of infertility in women with polycystic ovary syndrome.
