普通外科患者血红蛋白释放试验的比较研究

摘　　要：目的研究一些普外患者血红蛋白释放试验(HRT)结果的差异,探讨其临床意义。方法普通外科住院患者,取静脉血做HRT,采用3种HRT定点释放技术:等渗室温HRT、等渗37℃HRT、低渗37℃HRT各项处理结束后,进行淀粉-琼脂糖混合凝胶薄层电泳,第一次电泳后,停电一段时间后继续第二次电泳,丽春红、联苯胺染色后观察血红蛋白的释放情况。结果①等渗室温HRT:8例患者中有1例出现阳性结果;②等渗37℃HRT:10例患者中有3例出现阳性结果;③低渗37℃HRT:同上10例患者中有5例出现明显阳性结果。结论各种类型HRT的结果不同,灵敏度有差异,临床意义也有区别,可以提供多种疾病信息。HRT的基本原理是观察不同状态下红细胞膜释放血红蛋白的能力,详细机制有待于进一步研究。Objective To study the hemoglobin release test in different general surgical patients,and to approach its significance. Methods Blood was drawn from general surgery patients and hemoglobin release test （HRT） was carried out under isotonic HRT at room temperatur, isotonic HRT at 37℃ ,and hypotonic HRT at 37℃ respectively. The blood sample was analyzed with starch -agarose mixed gel electrophoresis. After first run of electrophoresis, the power was cut down for a while, the second run of electrophoresis was then performed. The gel was stained with ponceau and benzidine to show the release of hemoglobin from RBC. Results HRT was found positive in 1 out of 8, 3 out of 10 and 5 out of 10 blood sample in isotonic HRT at room temperature, isotonic HRT at 37℃ and hypotonic HRT at 37℃ respectively. Conclusion HRT can indicate the ability of red blood cell membrane to release hemoglobin under different states. The observation suggests that various types of HRT show different sensitivity and clinical significance and may provide a wide range of disease information.
