摘  要:为了克服传统立体定向术盲目穿刺导致出血的缺点,我们进行了Nd:YAG激光在内窥镜立体定向神经外科应用的实验及初步临床研究。实验研究显示Nd:YAG激光汽化范围极有限,深在凝固作用显著,冲水对减轻水肿有一定作用。在动物实验的基础上将激光应用于内窥镜立体定向手术治疗了9例脑深部及功能区病变,其中切除脑肿瘤6例;去除脑炎性肉芽肿2例;脑寄生虫摘除1例;所有病例进行了内窥镜直视下的活检,Nd:YAG激光用于术中止血及汽化、凝固病变组织,取得较好的临床效果。认为此种手术具有创伤小。This article described the animal experiment and primary clinical application of Nd: YAG laser in endoscopystereotactic neurosurgery. The experiment shows that vaporization of Nd: YAG laser is so limited and necresis is significant. Irrigation is benifit for decreasing the laserinduced edema. On the basis of animal experiment, Application of laser in endoscopystereotactic neurosurgery had been underdone in 12 patients whose lesions located in deepsited and functional areas since September, 1994. Brain tumors were resected totally or partly in 6 cases. Abcceses were acpirated and removed in 2 cases. Paraslte in 1 case and intracranial foreign metal bodies in 3 cases were extracted. Biopsies were made under direct visualization in all cases. Good results were obtained. It is considered that ELS has advantages of minimal invasion, direct intraoperative visualization and less complications. 
