北京地区静脉输液专业化发展现状调查及对策

摘　　要：目的了解目前北京地区静脉输液专业化发展现状。方法采用方便取样的方法,对北京地区47家医院进行问卷调查。结果①静脉治疗专业化情况:43家(91.5%)开展了经外周静脉置入的中心静脉导管(PICC)工作;仅有17家(36.2%)建立了静脉治疗专业小组,其中3家开展了静脉治疗护士的培训。②静脉配制的相关问题:9家(19.1%)成立了静脉药物配置中心;仅7家是由配液中心完成胃肠外营养液(TPN)的配制。③静脉输液相关的职业防护:40家(85.1%)在化疗药物配制间没有明确标示;39家(83.0%)尚未实行肿瘤化疗药物的集中配制和供应;27家(57.4%)尚未实现抗肿瘤药物配制人员定期的健康体检;21家(44.7%)尚未制定化疗药物配制的防护标准。结论北京地区静脉输液专业化发展现状不容乐观,静脉配制及职业防护方面也存在较多的不足,提示护理管理部门及相关医疗机构应积极采取相应措施推动北京地区静脉输液专业化发展的进程。Objective To investigate the status of intravenous infusion therapy in Beijing. Methods A questionnaire survey was conducted on 47 hospitals in Beijing. Results PICC （peripherally inserted central catheter） was implemented in 43 hospitals,which accounted for 91.5% in the sample. Among which,17（36.2%） hospitals set up intravenous infusion therapy teams and 3 hospitals provided nurse training for infusion therapy; The intravenous infusion drug deploy centers were built in 9 （19.1%） hospitals and TPN was prepared in intravenous infusion drug deploy centers in 7 hospitals. Regarding the occupational protection in infusion therapy,markers were not found in chemotherapy drug deploy room in 40 （85.1%） hospitals; Chemotherapy drugs were not deployed and supplied centrally in 39 （83.0%） hospitals; Routine physical examination was not provided for the personnel deploying chemotherapy drugs in 27 （57.4%） hospital; Occupational protection guidelines for chemotherapy drugs deployment were not established in 21 （44.7%） hospitals. Conclusions There are some problems in the current infusion therapy,intravenous drug deployment and occupational protection in Beijing. The department of nursing man- agement and relative medical institutions should take action to promote the development of infusion therapy in Beijing.
