HRCT与螺旋CT重建技术对慢性中耳炎诊断价值

摘　　要：目的探讨HRCT与螺旋CT图像重建技术对颞骨中耳结构情况反映的差别,并与手术中所见情况进行对比,评估其诊断价值。方法选择59例慢性化脓性中耳乳突炎行颞骨HRCT检查,显示中耳情况并与术中所见情况进行对比;再选择20例慢性化脓性中耳乳突炎分别对听小骨行多向调整多平面重组(MPR)及表面阴影成像(SSD)多层面切割技术;对面神经、半规管行曲面重建(CPR),以显示病变破坏情况并与术中所见情况对比;以术中所见到的真实情况为金标准,分析资料、统计数据。结论对慢性中耳乳突炎听小骨的诊断,MPR的特异性、敏感性、诊断的准确率高于SSD、HRCT;CT三种影像技术对听小骨全程显示的程度,MPR优于SSD、HRCT;对镫骨的诊断价值均较弱,MPR效果优于SSD、HRCT;CPR技术对面神经的诊断明显优于HRCT,与实际相符合率高;CPR技术对半规管的诊断价值尚不能得出结论。Objective To observe diagnosing value of HRCT and spiral CT image on chronic otitis media reconstruction technology. Methods 50 patients with chronic otitis media were examined with high resolution CT （HRCT） and be contrasted with operative findings. 20 patients with chronic otitis media were examined with CT image technology, high resolution CT （HRCT）, surface shaded display （SSD）, multi-planner reformation（MPR） for the ossicle and the curved face reconstruction （CPR） for the facial nerve and semicircular canna1, which were contrasted with operative findings. Results The overall ossicle, the sensitivity and specificity of MPR were much better than that of SSD and HRCT. About reflecting the overall ossicle, MPR was more clear and accurate than that SSD and HRCT. Conclusion The diagnostic value to stapes of the three image technology is not better enough and MPR is better than SSD and H RCT
