正清风痛宁治疗特发性局灶节段性肾小球硬化  

Assisting role of Zhengqing Fengtongning tablet in the treatment of idiopathic focal segmental glomerulosclerosis

摘　　要：目的探讨正清风痛宁辅助皮质类固醇激素联合环磷酰胺(CTX)治疗特发性局灶节段性肾小球硬化(I-FSGS)的临床疗效及不良反应。方法选择30例I-FSGS患者随机分为2组各15例。治疗组给予正清风痛宁辅助皮质类固醇激素联合CTX,对照组给予皮质类固醇激素联合CTX。比较2组3,6,9个月24 h尿蛋白定量、血清白蛋白、肌酐清除率的变化。结果治疗3个月后,治疗组和对照组均较治疗前24 h尿蛋白定量明显减少(P<0.05),血清白蛋白和肌酐清除率均明显升高(P<0.05)。6个月及9个月时,治疗组与对照组相比,24 h尿蛋白定量明显减少(P<0.05),血清白蛋白和肌酐清除率明显升高(P<0.05),总缓解率亦显著增加(P<0.05)。治疗组主要不良反应为消化道症状3例,肝脏损害2例,骨髓抑制2例;对照组主要不良反应为消化道症状2例,肝脏损害2例,骨髓抑制2例。结论正清风痛宁辅助皮质类固醇激素联合CTX治疗I-FSGS临床疗效优于皮质类固醇激素联合CTX,且不良反应无增加。Objective It is to approach the assisting role and side effect of Chinese Medicine Zhengqing Fengtongning tablet in the treatment of idiopathic focal segmental glomerulosclerosis （I - FSGS） with corticosteroid and Cytoxan. Methods 30 patients with I - FSGS were randomly divided into two groups with 15 cases each other. The treatment group was treated with Zhengqing Fengtongning tablet assisting with corticosteroid and Cytoxan; the control group was treated with corticos- teroid and Cytoxan. Then the values of 24h Urine protein quantitation, seralbumin, creatinine clearance rate between the two groups were detected and compared after therapy 3, 6, 9 months. Results The values of 24h Urine protein quantitation were decreased obviously in both groups after therapy 3 months, while the values of seralbumin and creatinine clearance rate were increased strongly （P〈 0.05）. After therapy 6 and 9 months, the values of 24h Urine protein quantitation were de- creased, and the values of seralbumin, creatinine clearance rate and remission rate were increased strongly in the treatment group（P 〈 0.05）. The main side effect included digestive symptom （3 cases in the treatment group, 2 cases in the control group）, liver impairment （2 cases in the treatment group, 2 cases in the control group）, myelosuppression （2 cases in the treatment group, 2 cases in the control group）. Conclusion Zhengqing Fengtongning tablet assist with corticosteroid and Cytoxan can be more beneficial in the treatment of I - FSGS.
