摘 要:目的探讨P16在成人中耳胆脂瘤上皮及中耳癌中的表达情况、作用及相互关系.方法应用免疫组化SP染色方法检测P16在30例成人中耳胆脂瘤上皮中及15例中耳癌患者肿瘤组织中的表达情况,应用计算机图像分析系统对其阳性表达进行定量分析.结果P16在成人中耳胆脂瘤上皮中阳性表达率分别为46.7%,与外耳道正常皮肤相比表达差异有统计学意义.P16在成人中耳胆脂瘤中的表达与侵袭能力之间无显关性(rs=-0.031,P=0.910).P16在成人中耳胆脂瘤中阳性细胞主要分布于上皮全层,以基底层和棘层为著,呈高度表达;而在对照组中阳性细胞仅在基底层表达,呈弱表达.P16在中耳癌中的阳性表达为50%,与外耳道正常上皮相比差异有统计学意义,与胆脂瘤相比差异无统计学意义.P16在中耳癌中的阳性细胞与胆脂瘤患者外耳道正常皮肤对照组相比染色呈强阳性,密度高.结论在成人中耳胆脂瘤及中耳癌中P16表达明显增高,说明P16的表达异常,使细胞无限增殖,P16在其中可能起到一个重要的作用.Objective To analyze the expression of P16 in human middle ear cholesteatomas and cancer, to investigate the correlation between its expression and the ability of erosion of cholesteatoma, and to analyze the role of P16 in proliferation of cancer. Methods The specimens from the acquired middle ear cholesteatoma tissue of 30 cases and 21 external ear canal skin samples from patients and 17 oxternal ear canal skin samples from healthy men and 15 cancer tissue samples from patients with cancer of the mierear were taken intraoperatively. The expression of Pl6 was examined by immunohistochemical S-P method. Then we scanned it into a computer by an image scanner and quantified the gray of them using commercial software. Results The percent of positive expression of P16 in middle ear cholesteatoma was 46.7%. Their expression was different from the skins of the control group. But there was no correlation between the expression of P16 and the ability of erosion of middle ear cholesteatoma (R = -0.031, P = 0.910). The expression of P16 in all pithelial layers of middle ear cholesteatoma were abundantly stained, especially in the basal and spinous layers. Only the basal layers were slightly stained in control group. The percent of positive expression of P16 in the cancer of the middle ear was 50%, There was sig-nificant difference between the cancer tissues and the cholesteatoma of middle ear. There wasn't correlation between the expression with P16 and ability of erosion in the cancer of middle ear. The staining of positive cells of P16 in the cancer tissues of middle ear was darker than the skin of external auditory meatus from control group, and the density was higher. Conclusions The expression of P16 in middle ear cholesteatoma and cancer tissues was up-regulated. It shows that the abnormal expression of P16 lead the proliferation of cells to be out of control, and may play an important role in the process of middle ear cholesteatoma and cancer.
