摘  要:背景:大鼠airpouch模型作为一种对骨科生物材料较敏感的体内模型,多用来检验磨损微粒刺激引起的细胞和炎症因子反应。而在人工关节置换的中晚期,磨损微粒可诱导假体周围组织细胞产生一系列生物学反应,导致假体周围骨溶解,最终产生无菌性松动。目的:评价应用大鼠airpouch模型进行磨损微粒造成无菌性松动机制分析的可行性。设计、时间及地点:随机对照动物体内实验,于2009-01/03在南昌大学第一附属医院中心实验室完成。材料:取SD大鼠30只,按随机数字表法分为3组,即Ti-6Al-4V微粒组、UHMWPE微粒组及生理盐水组,每组10只。另外,取人工髋关节无菌性松动假体周围界膜组织设计为界膜组。方法:大鼠背部皮下注射过滤后的空气3mL,每2d1次,共6次。1周后各组大鼠囊腔内分别注射Ti-6Al-4V微粒悬液、UHMWPE微粒悬液及生理盐水。主要观察指标:注射1周后处死动物,切开分离囊腔组织进行大体及光镜下观察,以免疫组织化学法检测白细胞介素6及肿瘤坏死因子α的表达。结果:大体观察发现,囊腔组织表面可见增生的小血管,部分区域可见纤维状组织,类似于无菌性松动假体周围界膜组织。2个微粒组光镜下可见大量吞噬细胞,界膜组出现同样的组织细胞反应,生理盐水组未见明显组织细胞反应。Ti-6Al-4V微粒组、UHMWPE微粒组及界膜组白细胞介素6的表达水平均高于生理盐水组(P<0.05),肿瘤坏死因子α的表达4组之间无明显差异(P>0.05)。结论:2种微粒均能刺激炎性细胞因子产生及活性升高,其效应类似于人工关节磨损微粒所引起的溶骨反应。提示大鼠airpouch模型能够代表磨损微粒所引起的生物学反应,是磨损微粒造成无菌性松动机制研究中较为可行的动物模型。BACKGROUND: Rat air pouch model, as in vivo models sensitive to bone biomaterial, has been widely used to measure cell and inflammatory factor reaction induced by arthroplastic wear particles. In middle and late stages of prosthetic replacement, arthroplastic wear particles can induce series of biological reaction of cells and osteolysis surrounding the prosthesis, finally resulting in sterile loosening. OBJECTIVE: To evaluate the feasibility of rat air pouch model to induce aseptic loosening due to arthroplastic wear particles. DESIGN, TIME AND SETTING: The randomized controlled animal in vivo study was performed at the Central Laboratory, First Affiliated Hospital, Nanchang University from January to March 2009. MATERIALS: A total of 30 Sprague Dawley rats were randomly assigned to 3 groups, Ti-6Al-4V particle group, UHMWPE particle group and saline group. In addition, limiting membrane surrounding the sterile loosening prosthesis of artificial hip joint served as limiting membrane group. METHODS: 3 mL of air following filtration was subcutaneously injected into rat back, once every two days, totally six times. Ti-6AI-4V particle, UHMWPE particle and saline were respectively injected into rats from the Ti-6Al-4V particle group, UHMWPE particle group and saline group 1 week later. MAIN OUTCOME MEASURES: 1 week following injection, animals were sacrificed. Capsular space was incised and generally observed under the optical microscope. Expression of interleukin-6 and tumor necrosis factor-α was detected by immunohistochemical method. RESULTS: General observation demonstrated that air pouch tissues situated in subcutaneous tissues, with small proliferous blood vessels in the surface, and fibroid tissues in some fields, similar to limiting membrane surrounding the prothesis tissues. Microscopy illustrated a large amount of phagocytes in the Ti-6Al-4V particle group and UHMWPE particle group, similar to the limiting membrane group.
