全膝关节置换50例:放置引流的随机对照比较

摘　　要：目的:比较全膝关节置换术后不放引流和放置引流对患肢周径、血红蛋白水平、膝关节功能和伤口愈合情况的影响。方法:选择中国医学科学院北京协和医院骨科收治的因晚期膝关节病变而行全膝关节表面置换术的患者50例,男12例,女38例,年龄33～76岁,平均64.2岁。根据置换后是否放置引流将患者分为不放引流组和放置引流组,每组25例,均常规行全膝关节置换术,并比较两组患者置换前和置换后1周的患肢周径(膝上10cm/膝下10cm)、血红蛋白水平、膝关节活动度及伤口愈合情况。结果:置换前、置换后1周不放引流组患者膝上10cm和膝下10cm患肢周径、血红蛋白水平、膝关节活动度与放置引流组相比,差异均无显著性意义(P>0.05)。不放引流组和放置引流组的所有病例,伤口均一期良好愈合,无脂肪液化、伤口感染等。结论:全膝关节置换术后放置引流管较不放引流并无明显优点。OBJECTIVE： To compare diameter of the affected lower extremity, hemoglobin level, function of the replaced knee joint and wound healing status after total knee arthroplasty （TKA） with or without a suction drainage. METHODS： A total of 50 patients with advanced knee joint diseases underwent TKA in Department of Orthopaedics, Peking Union Medical College Hospital, Chinese Academy of Medical Sciences, including 12 males and 38 females, aged 64.2 years （ranging 33 76 years）. The patients were divided in two groups： suction drainage group and no suction drainage group with 25 cases in each group. All patients underwent TKA routJnely. The dJameters of the affected lower extremities （10 cm above and below the knee joint）, hemoglobin levels, ranges of motion （ROM） of the replaced knee joints preoperatively and one week after TKA and wound healing status of two groups were compared. RESULTS： Diameters of the affected lower extremities 10 cm above and below the knee joint, hemoglobin levels, ROM of the replaced knee joints in no suction drainage group was similar to suction drainage group （P 〉 0.05）. The wound of all patients in two groups was healed at the primary stage with no fat liquescence or infection. CONCLUSION： Replacement of suction drainage in TKA has no obvious advantage compared with TKA without a suction drainage.
