摘 要:背景:目前临床上常把股骨颈干角冠状面投影角作为股骨颈干角的参考,影像学上亦没有股骨颈干角的精确测量方法。目的:探讨一种精确的股骨颈干角计算方法。设计、时间及地点:探索性实验,于2008-10在山东省单县中心医院完成。材料:股骨颈标本由泰山医学院解剖教研室提供。方法:以股骨颈基底部中心点O为坐标中心建立三维坐标系,设股骨颈前倾角为α,股骨颈干角为β,股骨颈干角冠状面投影角为β’,对股骨颈前倾角和股骨颈干角及其冠状面投影角相互关系进行数学研究,推导股骨颈干角计算公式。主要观察指标:股骨颈干角,股骨颈前倾角,股骨颈干角投影角。结果:股骨颈干角β=90°+arctg[tg(β’-90°)cosα]。结论:该公式简单,准确,值得临床参考。BACKGROUND: Projection angle of femoral neck-stem angle at coronal plane is clinically regarded as reference of femoral neck-stem angle. There has been no precise measurement method for the femoral neck-stem angle in imaging field. OBJECTIVE: To explore an accurate calculation method of the femoral neck-stem angle. DESIGN, TIME AND SETTING: The study was performed at the Shanxian County Central Hospital of Shandong Province in October 2008. MATERIALS: Samples of the femoral neck were provided by Department of Anatomy, Taishan Medical University. METHODS: 3D coordinate system was established with the center of femoral neck base as coordinate center; the anteversion angle of femur neck was a, femur neck-stem angle was [3, and projection angle of femur neck-stem angle on coronal plane was 13 The relationship among anteversion and femoral neck-stem angle and its projection angle on coronal plane was explored to deduce a calculating formula of the femoral neck-stem angle. MAIN OUTCOME MEASURES: Anteversion of femur neck, femur neck-stem angle, and projection angle of femur neck-stem angle on coronal plane. RESULTS: Femoral neck-stem angle β=90° +arctg[tg(β′-90°)cosa]. CONCLUSION: This formula is simple and accurate and could serve as clinical reference.
