电视纵隔镜在胸部疑难疾病中的临床应用

摘　　要：目的探讨电视纵隔镜检查术在胸部疑难疾病并纵隔淋巴结肿大患者的临床应用价值。方法对2005年10月～2008年3月在该院胸外科住院的39例胸部疑难疾病并纵隔淋巴结肿大患者行电视纵隔镜检查并进行活检获取病理以明确诊断。结果手术经过顺利,无严重并发症及围手术期死亡。39例患者均获明确病理诊断,其中,肺腺癌12例,肺鳞癌8例,肺小细胞癌6例,恶性淋巴瘤6例,纵隔淋巴结结核4例,结节病2例,纵隔慢性淋巴结炎1例。结论电视纵隔镜手术创伤小,安全,取材可靠,可作为胸部疑难疾病并纵隔淋巴结肿大的患者的有效诊断方法。[Objective] To investigate clinical application value of video-mediastinoscopy in the diagnosis of undetermined thoracic diseases with mediastinal lymphadenectasis. [Methods] Thirty-nine cases of undetermined thoracic diseases with mediastinal lymphadeneetasis underwent biopsy by video-mediastinoseopy from October 2005 to March 2008 to make sure the diagnosis. [Results] There was no severe complication or death. All patients had definitive diagnosis after operation, among which adenocarcinoma of lung was diagnosed in 12 patients, lung squamous carcinoma in 8, small cell carcinoma of lung in 6, lymphoma in 6, tuberculosis in 3, sarcoidosis in 2 and chronic lymphadenitis in 1. [Conclusion] Video-mediastinoscopy with little operation wound is a safe and effective procedure for diagnosis of undetermined thoracic diseases with mediastinal lymphadenectasis.
