内镜作深筋膜下超声刀交通静脉离断术(SEPS)治疗静脉溃疡

摘　　要：目的探讨内镜作深筋膜下超声刀离断交通支静脉治疗下肢慢性静脉性溃疡的疗效。方法手术治疗12例下肢静脉曲张患者,该12例均同时存在浅静脉倒流、交通静脉功能不全、深静脉瓣膜功能不全、静脉性溃疡。手术方式为内镜作深筋膜下超声刀离断交通支静脉、大隐静脉高位结扎并剥脱和小腿曲张的浅静脉激光闭塞。结果术后肢体症状和浅静脉曲张消失,11例静脉性溃疡短期内愈合,1例明显缩小。结论腔镜深筋膜下超声刀交通支静脉离断术安全有效,损伤小,并发症少,是治疗下肢静脉功能不全性溃疡的有效方法。Objective To evaluate the role of subfaseial endoscopic perforator ligation by using ultra- sonic scalpel in treatment of patients with venous ulceration of the leg. Methods 12 patients with venous ulcer in the lower extremities ,who had been identified to have reflux in superficial and deep vein as well as incompe- tent perforating vein , underwent venous surgery. Several surgical procedures were performed simultaneously , including subfasieal endoscopic perforator surgery by using ultrasonic scalpel , endovenous laser treatment, smaller incisions and stripping of the long saphenous . Results The surgery obviously improved venous disability , varicose vein disappeared ;ulcers healed shortly in 11 cases and ulcer reduced markedly in another one case. Conclusion Subfaseial endoscopic perforator ligation by using ultrasonic scalpel is a safe and effective technique. It plays an important role in the treatment of varicose and venous ulceration in leg.
