摘  要:对连续行左右心导管检查的41例风心二尖瓣狭窄患者采用多平面食管超声心动图检查(TEE),结果表明,TEE压力减半时间、血流会聚法能准确定量二尖瓣狭窄,血流面积法定量二尖瓣狭窄的准确性较差;多平面TEE于0°、45°、90°和135°扫描切面均可准确定量狭窄的二尖瓣口面积;约60%患者TEE所测经二尖瓣最大压力阶差低估实际值。The aim of this study was to compare the accuracy of pressure half time with proximal flow convergence and flow area methods in evaluation of mitral valve area(MVA)by transesophageal echocardiography(TEE)in patients with rhumatic mitral stenosis,and to understand the feasibity of measuring hemodynamic parameters by TEE.The results showed that(1)MVA determined by pressure half time and proximal flow convergence method correlated well with that by Gorlins formula(r=0 87,and 0 88 respectivly),the flow area method provided a lower correlation(r=0 51)and underestimated the MVA.(2)The pressure half time MVAs from any of the 0,45,90,135 degree imaging plane correlated well with that by Golins formula(r=0 81,0 79,0 87,0 83).(3)TEE Doppler derived maximum pressure gradients(max PGs)across the stenotic mitral valve orifice underestimated the catheterization values of max PGs in sixty percent of patients,but TEE Doppler mean pressure gradients(meanPGs)correctively evaluated the catheterization meanPGs.
