特发性肺含铁血黄素沉着症临床及X线分析

摘　　要：目的：探讨ＩＰＨ胸部影像特点，及其病理基础和在诊断中的价值。方法：回顾性分析３６例特发性肺含铁血黄素沉着症（ＩＰＨ）１～８ａ追踪资料及１１２例次胸片。结果：本组首次胸片诊为ＩＰＨ者１８／３６例（５０％），误诊１５／３６例（４１％）；３例正常。胸部Ｘ线改变为呈进行性，初步分为四型：毛片型和间质浸润型，为早期表现可于短期内吸收；不可逆的网粒型及后遗症型。结论：结合典型临床表现。Objective:To explore the X-ray characteristics of chest of IPH,its pathologic basis and diagnostic value.Mehtods:The information of 36 children with idiopathic pulmonary hemosiderosis(IPH) followed up for 1～8 years and 112 X-ray films of chest were retrospectively reviewed.Results:18 cases(50%)were diagnosed 15 cases(41%)were misdiagnosed and 3 cases were normal at primary time.X-ray manifestations were divided into four types:the frosted glass type and interstitial infiltrative type were early manifestations and could be absorbed in short time;inreversible reticulonodular type and sequela type.Conclusion:To anlyse the specific X-ray features in detail combined with typical clinical manifestation will be help for diagnosis and differential diagnosis of IPH.
