PtfV_1对二尖瓣球囊扩张术疗效的评价

摘　　要：为了探讨风湿性心脏病二尖瓣狭窄患者经皮二尖瓣球囊成形术（ＰＢＭＶ）前后心电图ＰｔｆＶ１的改变与疗效的关系，本文分析了２１０例窦性心律的二尖瓣狭窄ＰＢＭＶ前后ＰｔｆＶ１的变化，并与血流动力学和心脏Ｂ超的有关参数作相关性研究，结果：ＰｔｆＶ１术前为－０．０８７±０．０３／ｍｍ．ｓ，术后－０．０４２±０．０２７／ｍｍ．ｓ（Ｐ＜０．０１），ＰＢＭＶ前后ＰｔｆＶ１的改变与平均左房压（ＭＬＡＰ），二尖瓣跨瓣压差（ＭＰＧ），左心房内径（ＬＡＲ）呈负相关；与二尖瓣口面积（ＭＶＡ）呈正相关，而与心率（ＨＲ）无明显的相关性，提示ＰｔｆＶ１的测定可作为ＰＢＭＶ后疗效评价及功能随访的一种可靠、无创。\ Objective: To study the change of PtfV 1 of ECG before and after percutaneous balloon mitral valvuloplasty(PBMV) and its correlation with effect in patient with rheumatic heart disease (RHD) Mitra stenosisMethod:we measured the PtfV 1 of ECG and obsened hemodynamics and echocardiography idic before and after PBMV in 210 sinus rhythmic patients with RHD and correlation studyResults show that PtfV 1 before and after was -0087±0031/mms and -0042±0027mms(P<001)The changes of PtfV 1 before and after PBMV present negative correlation with left atrial pressure (LAP) and mitral pressure gap(MPG) and left atrial diameter(LAR),while present positive correlation with mitral valve area(MVA),but was not correlated with heart rate(HR)It is suggested that PtfV 1 may be as a reliable and noninvasive and economical method that evaluate the effect of PBMV and follow up cardiac function \ \
