高位腰椎间盘突出症的诊断与治疗

摘　　要：本文就收治的１５例高位腰椎间盘突出症进行了分析讨论，其中Ｌ１、２突出２例，Ｌ２～３突出３例，Ｌ３～４突出１０例，皆经ＣＴ或ＭＲＩ确诊并经手术证实。手术采用半椎板或全椎板切除髓核摘除术。术后恢复优良者１２例，中等者２例，１例无变化。文中就高位腰椎间盘的解剖特点、临床表现和诊断特点及治疗进行了讨论，并指出ＣＴ、ＭＲＩ应是首选的影像学检查手段。This article reported 15 cases of protrusion of lumbar intervertebral disc in high position, including L 1 2 in 2 cases, L 2 3 in 3 and L 3 4 in 10 cases. All patients were diagnnosed by CT or MRI, and verified in operation. Nucleus puiposus were removed by semi laminectomy or omni laminectomy. After the operation, the outcome were excellent in 12 cases, fair in 2 and no change in 1 case. The anatomical features, clinical manifestation, diagnostic characteristic and treatment of protursion of lumbar intervertebral disc in high position was discussed in this article. It was suggested that CT and MRI were the first method to make the diagnosis, and that early diagnosis and timely operation were the key to get satisfactory results of treatment.
