激素性股骨头坏死动物模型建立方法初探

摘　　要：目的:研究激素性股骨头坏死(steroid-induced necrosis of femoral head,SNFH)动物模型的建立方法。方法:12只健康雄性新西兰白兔随机分成甲基泼尼龙组(M组)、甲基泼尼龙+抗生素组(MA组)和对照组(C组),M组和MA组均每周2次,每次肌注甲基泼尼龙琥珀酸钠8 mg/kg,MA组每只每周2次青霉素40万U(0.24 g)肌注,经常性增加动物后肢受力的负荷,激素注射8周后进行股骨头CT和组织病理检查,观察造模效果。结果:M组在激素注射后5周内全部死亡,MA组建模期间全部存活。CT显示MA组无特征性股骨头坏死表现,但局部结构模糊不清,有高亮度影。MA组病理检查肉眼可见股骨头软骨面局部缺损,坏死面暴露,髓腔内大量脂肪组织堆积;光镜下成骨细胞、骨细胞均减少,空骨陷窝增多,微小血栓广泛分布,变形扩大的脂肪细胞挤压正常骨髓组织细胞,骨小梁变细破裂明显。结论:本实验建立SNFH动物模型的方法有效,建模过程必须同时给予适量的抗生素类药物预防感染,CT作为早期股骨头坏死(necrosis of femoral head,NFH)影像学观察和评价手段有一定局限性。Objective： To study the establishment of animal models of steroid - induced necrosis of femoral head. Methods： Twelve healthy New Zealand male rabbits were randomly divided into methylprednisolone group（MG）, methylprednisolone ＋ antibiotic gmups（MAG）and controlled groups（CG）. The rabbits of MG and MAG were IM methylprednisolone sodium succinate 8 mg/ kg twice a week. MAG IM benzylpenicillin sodium 0.24 g twice a week yet. Increased the weight loading on the hind limbs of rabbits. CT and histology examination of femoral heads would be done after 8 weeks. Then, effects of the method were observed. Results： After 5 weeks of injection, alll of the rabbits in MG dead but MAG were living. CT displayed the structure of femoral head was vague. A piece of coloboma could be observed clearly on the cartilage of a femoral head. Medullary canal was almost filled with white adipose tissue. Osteoblasts and osteocytes decreased. Empty bone lacuna increased. Thrombus distributed widely in the bone tissue microcirculation. Many big adipocytes displaced the normal marrow tissue. Bone trabeculas were thin and broken. Conclusion： The establishment of animal models of SNFH is successful. Some kinds of antibiotic should be injected at the ,same time. CT isn＇t the best way to observe the early stages of NFH.
