出院后干预对乳腺癌患者心理状态生活质量的影响

摘　　要：目的探讨出院后干预对乳腺癌患者生活质量、心理状态的影响。方法随机选择本中心住院治疗的乳腺癌患者90例,分为两组,各45例,均给予常规护理和治疗。观察组在出院后持续半年以上参加"内分泌治疗随访"和乳癌病友俱乐部——"汝康会"活动。对照组经医院治疗后,至少半年未参加以上活动。在出院时、出院后2个月、出院后6个月对两组进行症状自评量表和生活质量核心问卷测评。结果出院时两组症状自评量表和整体生活质量的得分差异无统计学意义(P>0.05)。出院后2个月,观察组的抑郁、焦虑和恐怖因子得分低于对照组(P<0.01或P<0.05);出院后6个月,观察组的人际关系敏感、抑郁、焦虑因子和总症状指数得分低于对照组(P<0.01或P<0.05)。在出院后2个月和出院后6个月,观察组在失眠、经济困难和整体生活质量因子上得分与对照组比较差异有统计学意义(P<0.01)。结论对乳腺癌患者实施出院后专项治疗随访和病友俱乐部活动能明显提高其生活质量,改善其心理状态。Objective To explore the influence of post-discharge intervention on breast cancer patient＇s psychology and life quality. Methods 90 randomly chosen patients with breast cancer hospitalized in the Center were divided into the observation group and the control group, each with 45. Both groups were offered the conventional treatment and nursing. The observation group were required to take part in activities of ＂Follow-up Visit for Endocrinopathy＂ and ＂Club of Fellow Patients＇ with Breast Cancer＂ for over half a year after discharge, while the control group were not within at least half year after the discharge. On discharge, two months and six months after discharge, the two groups were rated by means of Symptom Checklist and Core Questionnaire on Life Quality. Results The scores for the two groups on discharge had no statistical significance （P〉0.05）; two months after discharge, the scores for the factors of depression, anxiety and terror of the observation group were lower than those for the control group （P〈0.01, P〈0.05）, and six months afterward, the scores for the factors of interpersonal sensitivity, depression and anxiety and the score for the overall symptom index were, too （P〈0.01, P〈0.05）. Two months and six months after discharge, the differences in the scores for the factors of insomnia, economic difficulty and overall life quality between the observation group and the control group had statistical significance （P〈0.01, P〈0.05）. Conclusion Activities of post-discharge follow-up visits to breast cancer patients and Club of Fellow Patients＇ with Breast Cancer can markedly improve the psychology and life quality of breast cancer patients.
