高容量血液滤过对重症腹腔感染术后MODS的治疗作用

摘　　要：目的:探讨高容量血液滤过(HVHF)在重症腹腔感染术后MODS患者治疗中的临床效果。方法:腹腔重症感染术后进行HVHF治疗的MODS患者(17例),观察治疗前后患者炎性介质(TNF-a、IL-6、IL-8)、氧合指数、APACHE-Ⅱ评分。结果:17例患者经HVHF首次治疗后APACHE-Ⅱ和TNF-a、IL-6、IL-8也有明显下降,氧合指数改善,差异均有统计学意义(P<0.05)。经多次治疗后,最终患者APACHE-Ⅱ评分明显下降(P<0.05)。结论:HVHF可以改善严重脓毒血症患者的呼吸功能及生命体征,并能缓解病情。AIM： To evaluate the clinical efficacy of high-volume hemofiltration （HVHF） on treating postoperative multiple organ dysfunction syndrome in severe abdomi- nal infection patients, METHODS： Data of 17 severe abdominal infection patients treated with HVHF were collected and the differences on oxygenation indices, APACHE-11 scores before and after HVHF therapy were observed. RESULTS： After the first HVHF treatment, the oxygenation index improved, and the APACHE-Ⅱ scores, TNF-a, IL-6, and IL-8 decreased significantly （P〈0.05）. After several HVHF treatments, the oxygenation index improved, and the APACHE-Ⅱ scores decreased significantly （P〈0.05）. CONCLUSION： HVHF can improve both the respiratory function and vital signs of severe sepsis patients and relieve the severity of the illness.
