慢性阻塞性肺疾病中多维评分系统的临床应用及评价

摘　　要：目的探讨多维评分系统（BODE）评分在慢性阻塞性肺疾病评价中的应用价值。方法对55例稳定期COPD患者FEVI、BODE评分，随访12个月并记录COPD危险事件（包括呼吸衰竭和死亡）、就诊次数、住院次数、机械通气次数，评估存活病例的SOLDQ记分、COPD急性加重（AECOPD）次数、住院时间等。结果FEV1≥1L组及BODE评分〈5分组，患者生活质量、死亡人数、就诊次数、AECOPD次数、AECOPD住院次数、机械通气次数、住院时间及SOLDQ评分均好于FEV1〈1L组及BODE评分≥5分组，差异有统计学意义（P〈0．05）。BODE评分与死亡例数、就诊频数、AECOPD住院次数、住院时间均呈显著正相关；而FEV1与住院次数、SOLDQ评分及机械通气次数呈显著负相关。结论BODE评分系统和FEV1均能评价COPD患者的病情严重程度，以BODE评分系统更为全面及准确。Objective To investigate the value of the multidimensional scoring system（BODE） score in patients with chronic obstructive pulmonary disease （ COPD ）. Methods Fifty-five stable COPD patients were evaluated for forced expiratory volume in first second（ FEV1 ） and BODE score, and recorded the risk of C OPD dangerous events （ including respiratory failure and death） , the number of visits, frequency of hospitalization and mechanical ventilation, assessed the SOLDQ score of survival cases, requency of COPD acute exaeerbation （AECOPD） ,length of stay in hospital for analysis. Results FEV1 ≥ 1 L group and BODE score 〈 5 subgroup, patients health and quality of life, the number of the death and the visits, frequency of AECOPD and AECOPD hospitalization, frequency of mechanical ventilation, hospitalization time and SOLDQ score were better than the FEVI 〈 1L group and BODE score ≥5 grouping,there were statistical significant difference （ P 〈 0.05 ）. There was significant positive correlated in BODE score and the number of death, the number of visits, frequency of AECOPD hospitalization, length of stay in hospital ; there was negatively correlatedand in FEV1 and the frequency of hospitalized, SOLDQ score, the frequency of mechanical ventilation. Conclusion Both FEV1 and BODE score can be used to evaluate disease severity in COPD, however, BODE score is better than FEV1.
