儿童不同年龄组肺炎支原体肺炎临床特点分析

摘　　要：目的了解不同年龄儿童肺炎支原体肺炎（MPP）的临床特征，为经验治疗提供依据。方法收集2007年1月～2009年1月本院确诊的195例MPP患儿，对其临床资料进行总结。分为婴幼儿组（0～3岁）和年长儿组（3-14岁），对比分析两组临床症状、体征、X线表现、肺外并发症情况及合并其他病原体方面的差异。结果婴幼儿组MPP主要症状及肺外表现与年长儿差异有统计学意义（P〈0．005）。婴幼儿组MPP咳痰喘息症状相对明显，肺部干湿罗音多，体温相对低，平均热程短：年长儿组多表现高热，干咳，症状重，肺部体征轻，平均热程长。MPP患儿胸部X线表现具多样性，小叶实质浸润是各年龄组最常见的胸部X线征象。年长儿组肺段实质浸润更多见，消化系统症状更少，以胸膜受累多见。结论不同年龄儿童MPP在临床症状、肺部体征及胸部X线表现等方面均存在一定差异．须结合实验学等检查才能帮助早期诊断。Objective To explore the characteristics of mycoplasma pneumoniae pneumonia in different ages children. Methods 195 children with MPP from Jan 2007 to Jan 2009 were divided into two groups according to ages as follows infants （〈3 years old）group ,older children （3-14 years old）group.The differences in children with MPP from different ages in terms of clinical symptoms pulmonary signs and chest X-rays extrapulmonary complications were compared. Results There were significant differences in main symptoms between two groups （P〈0.005）. The infants group wet coughs,wheezing and rales were more severe,but the temperature was lower and fever period was shorter compared with the old groups ,whereas the majority of older group showed high fever,severe nonproductive coughs,slight pulmonary signs and longer fever time . The children with MPP had various manifestations in chest X-rays. Lobular parenchymtous infiltration was most common findings in two groups. The segmental parenchymtous infiltration was more common in older group. Less digestive sympotoms and more pleural abnomalities could be found in older ages commonly. Conclusion There are differences in clinical symptoms pulmonary signs and chest X-rays in MPP children from different age groups. Experimental tests including serological and other tests should be done to help early diagnosis.
