自制匀浆膳在慢性阻塞性肺疾病患者机械通气中的应用价值

摘　　要：目的探讨管饲匀浆膳对慢性阻塞性肺疾病(COPD)并发呼吸衰竭患者机械通气的营养支持效果。方法随机将60例COPD患者分为对照组(n=30)与治疗组(n=30),两组均给予抗感染、祛痰、解痉、平喘、纠正水电解质紊乱等常规治疗。对照组给予能全素鼻饲,治疗组给予自制匀浆膳分次胃内注入,并监测相关数据。结果治疗组较对照组在提高血液中血红蛋白(Hb)、血清总蛋白(TP)、白蛋白(ALB)、外周血淋巴细胞(TCL)含量及降低血尿素氮(BUN)更明显,并能减少并发症的发生率,缩短机械通气及入住ICU时间,减少医疗费用。结论对COPD行机械通气的患者进行肠内匀浆营养更安全、方便、经济、有效。Objective To investigate the impact of tube feed of serf - made homogenate diet on the nutritious supplements in COPD patients received mechanical ventilation. Methods Sixty patients were randomly divided into control group （ n = 30） and treatment group （ n = 30）. All the patients in two groups were given the same routine treatment of anti - infection, expectorant, relieving spasm, antiasthma, balancing hydro - electrolytic disorder etc. The control group was given tube feed of nutison, the treatment group was given homogenate diet. The relevant data was monitored. Results The treatment group is more obvious to improve Hb, TP, ALB, TCL and decline BUN. There were significant differences to reduce the complications and cost of the nutrition, short the mechanical ventilation time and in ICU stay time. Conclusion Homogenate diet is more convenient, economical, effective and safer than that of nutrison for COPD patients with mechanical ventilation.
