子痫前期及子痫患者血小板膜糖蛋白Ibα、Ⅱb、PLT及其相关参数的变化

摘　　要：目的观察子痫前期及子痫患者血小板膜糖蛋白Ibα(GPIbα)、Ⅱb(GPⅡb)、血小板计数(PLT)及其相关参数的变化。方法采用流式细胞术检测32例重度子痫前期及子痫患者、16例轻度子痫前期患者、22例孕晚期妇女剖宫产术前及剖宫产术后72h内GPIbα和GPⅡb水平的变化;同期采用全自动血细胞分析仪检测PLT及其相关参数-平均血小板体积(MPV)及血小板分布宽度(PDW)。结果剖宫产术前,与轻度子痫前期组、正常孕晚期组相比较,重度子痫前期及子痫组GPIbα及PLT明显降低(P<0.01);MPV及PDW明显升高(P<0.01),GPⅡb变化差异无显著性(P>0.05)。剖宫产术后,各组GPIbα、PLT及其相关参数均恢复至剖宫产术前正常孕晚期水平,GPⅡb变化差异无显著性。结论孕晚期重度子痫前期及子痫患者存在血小板活化、消耗状态,GPIbα、PLT及其相关参数可作为诊断监测指标。产后GPIbα、PLT及其相关参数较快恢复至正常孕晚期水平可能与胎盘娩出有关。[Objective] To observe the changes of Platelet GPIbα, GP Ⅱb, PLT and platelet parameters during late gestation in patients with severe pre-eclampsia and eclampsia. [Methods] Levels of GPIbα and GP Ⅱb were deteceted by flow eytometcy （FCM）; PLT, MPV, and PDW were detected by blood cell automatic analyse instrument. The venous blood of 32 severe pre-eclampsia or eclampsia women, 16 mild pre-eclampsia women and 22 normal lale pregnany women were collected before operation and 72 hours after the operation. [Results] Before the cesarean section, levels of GPIbα and PLT of severe pre-eclampsia and eelampsia patients are obviously lower （P〈0.01） and levels of MPV and PDW are obviously higher （P 〈0.01） than those of mild pre-eclampsia and normal pregnancy women. After cesarean section, levels of GPIbα, PLT and platelet parameters are all returned to levels of late pregnancy before cesarean section. GP Ⅱb have no obviously change. [Conclusion] Patients with severe pre-eclampsia and eclampsia have the condition that the platelets are activated and consumed,GPIbα, PLT and Platelet parameters can be important markers of this disease. Levels of GPIbα, PLT and platelet parameters after cesarean section return to levels of late pregnancy maybe concerned with the expulsion of placenta.
