摘  要:目的:了解老年糖耐量减低患者不同血糖水平与高敏C反应蛋白之间的相关性。方法:308例老年非糖尿病患者,根据口服葡萄糖耐量试验结果分为糖耐量正常组80例及糖耐量减低组(研究组)228例,再根据餐后2 h血糖水平将研究组分为低高血糖组85例,中高血糖组72例及高高血糖组71例。测定各组身高、体质量、血压、血脂、血清C反应蛋白、空腹胰岛素等生化指标并比较分析。结果:低密度脂蛋白对照组低于糖耐量减低各组,而高密度脂蛋白对照组高于糖耐量减低各组,对照组与糖耐量减低各组比较差异均有统计学意义(P<0.05),但糖耐量减低3组间无论血糖水平高低差异均无统计学意义(P>0.05);高高血糖组高敏C反应蛋白及胰岛素抵抗指数水平最高,分别为(1.69±0.93)mg/dL,(2.60±2.06)mg/dL,与其他各组比较差异均有统计学意义(P<0.01),而对照组、低高血糖组、中高血糖组组间比较差异无统计学意义(P>0.05)。相关分析显示高敏C反应蛋白与餐后2 h血糖呈正相关。结论:糖耐量减低患者已存在脂代谢异常,餐后2 h血糖为10.0~11.1 mmol/L的老年糖耐量减低患者是动脉粥样硬化的高危人群,需早期进行生活方式干预和药物治疗。Objective To evaluate the relationship between high sensitivity C-reactive protein and different glucose levels in the senile patients with impaired glucose tolerance. Methods Three hundred and eight patients were divided into four groups, including normal glucose tolerance group (n=80), mild impaired glucose tolerance group (n = 85), moderate impaired glucose tolerance group (n = 72), sever impaired glucose tolerance group(n=71) according to OGTT results. The body height, body weight, blood pressure, blood lipids, serum insulin and high sensitivity C-reactive protein were detected. Results The level of LDL was lower in control group than IGT groups. The HDL level was higher in normal group than IGT groups(P〈0.05). There was no difference in IGT groups. The levels of high sensitivity C-reactive protein and insulin resistance index were highest in IGT 3 group, respectively (1.69 ± 0. 93) and (2. 60±2. 06) mg/dL, statistically different from the other three groups(P〈0.01). And there was no statistic difference in other three groups(P〉0.05). The high sensitivity C-reactive protein was positivly related to 2-hour blood glucose. Conclusion The patients with impaired glucose tolerance have lipid metabolism disorders. The senile patients with impaired glucose tolerance and 2-hour blood sugar of 10. 0 to 11. 1 mmol/L are in high danger of atherosclerosis, and should be intervened and treated as early as possible.
