摘 要:目的探讨高分辨磁共振T1WI(HR-T1WI)技术参数优选在显示胸壁结构及病变中的应用。资料与方法5名正常自愿者和45例胸膜病变患者行常规T1WI、T2WI及HR-T1WI平扫,患者再行常规T1WI增强及HR-T1WI增强扫描。对HR-T1WI不同技术参数成像的图像在胸壁结构及病变显示清晰度方面进行比较。结果对胸壁结构及病变显示清晰度评分达到3分者,Torso线圈和5in表面线圈分别占87%、40%,激励次数4和激励次数2分别为85%、40%,采集矩阵512×256和256×256分别为85%、35%(P<0.05);另外视野18cm×18cm与12cm×12cm分别为80%、60%;层厚/层间距3/0.5(mm)、2/0.5(mm)分别为86%、80%(P>0.05)。结论HR-T1WI提高了图像空间分辨率及具有较高信噪比,可作为胸部常规扫描的重要补充序列。采用5in表面线圈、视野≤18cm×18cm、层厚/层间距≤3/0.5(mm)、采集矩阵≥512×256和激励次数≥4时,对胸壁结构及病变显示较满意。Objective To explore the role of High Resolusion (HR) MRI T2WI technique in displaying chest wall structure and pleural diseases. Materials and Methods Five normal volunteers and 45 patients with pleural diseases were examined with SET1 ,FSE T2 and HR-T1WI scanning. The patients were further examined with HR-T1WI contrast and T1WI contrast MR scanning. The images of displaying chest wall structure under different scanning parameters of HR-T1WI were compared. Results The images of HR-T1WI with a loop of 5 inch surface loop were better than that with a loop of Torso loop ( P 〈0.05 ) . The HR-T1WI images with a matrix of 512 × 256 were better than that with a matrix of 256 × 256 ( P 〈 0. 05 ). The degree of displaying chest wall structure on HR T1WI has no significant difference between using a field of 18 x 18 centimeter and 12× 12 centimeter ( P 〉0. 05 ). The degree of displaying chest wall structure on HR-T1WI has no significant difference between using a layer of 2/0.5 millimeter and 3/0.5 millimeter( P 〉 0.05 ). Conclusion With high spatial resolution and CNR, HR-MRI T1WI can be applied in the diagnosis of pleurai diseases as a supplementary sequence. Using the parameters as 5 inch surface coil, layer ≤3/0.5, field ≤ 18 × 18, matrix ≥512 × 256,and excitation number ≥4, HR-MRI reveals deep layer structure of chest wall and pleural diseases perfectly.
