摘 要:目的通过与常规Subtraction软件比较,探讨NeuroDSA软件在16层螺旋CT颈动脉减影去骨成像中的应用价值。资料与方法21例疑颈部动脉病变患者,小剂量测试后确定动脉期延迟时间,行相同范围低剂量平扫与颈动脉CTA,分别进行Subtraction减影去骨及NeuroDSA减影去骨成像,比较两种去骨方法的成像时间及手工辅助去骨前后的图像质量。结果21例颈动脉平均强化值为(396.58±97.71)HU;颈动脉与颈静脉平均强化差值为(231.33±94.85)HU;颈静脉干扰轻微,1例静脉返流严重,1例位置移动过大不能进行减影去骨血管成像。其余19例均可进行减影去骨处理,Subtraction软件直接减影的平均时间及手工辅助去骨平均时间分别是(4.05±1.28)min、(20.95±6.73)min,NeuroDSA减影的平均时间及手工辅助去骨平均时间分别是(2.35±1.05)min、(8.39±4.03)min。两者差异有统计学意义(P<0.001)。Subtraction软件直接去骨及手工辅助去骨后图像评分分别为1.08±0.28、2.15±0.69。NeuroDSA软件直接去骨及手工辅助去骨后图像评分分别为2.46±0.78,2.85±0.38。两者差异有统计学意义(P<0.05)。21例共发现动脉瘤3例,动脉狭窄12例。两者均能清晰显示所有病变。结论NeuroDSA软件较常规Subtraction软件能更快速、自动去除颈动脉CTA中的骨结构,更清晰、全面显示颈动脉血管,具有较高的应用价值。Objective To investigate the value of Neuro DSA software in Cervical Arteries CT angiography (CTA) compared with subtraction software. Materials and Methods The delay scanning time of MSCT carotid angiography was definited by Test-Bolus. 21 patients underwent low dose MSCT plain scan and CT angiography with the same scanning range. Excepting one case of contrast medium back streaming in the vein and one case of movement, there were 19 cases were collected. The bone removal was post processing reconstructed using subtraction software and Neuro DSA software re- spectively.. The processing time and image quality of the two bone removal softwares were compareed. Results The mean arterial attenuation value and mean attenuation difference between artery and vein were 396.58 ± 97.71HU and 231.33 ± 94.85HU. The automatic processing time and the hand-made processing time were longer with subtraction software versus with Neuro DSA software( 4.05 ± 1.28min and 20.95 ± 6.73 VS 2.35 ± 1.05min and 8.39 ± 4.03min, respectively 〈 0. 001 ). The scores of image quality of automatic bone removal and bone removal with hand-made were lower with subtraction software versus with Neuro DSA software ( 1.08 ± 0.28 and 2.15 ± 0.69 VS 2.46 ± 0.78 and 2.85 ± 0.38, respectively 〈 0.05 ). Aneurys ms in 3 patients and arteriostenosis in 12 patients were comparably demonstrated with the two kinds of software. Conclusion The cervical bone structures were rapidly and automatically removed and the cervical arteries were displayed more dearly with Neuro DSA software.
