新生儿与成人金黄色葡萄球菌感染耐药谱的比较

摘　　要：目的：探讨新生儿金黄色葡萄球菌感染的耐药谱。方法：根据临床资料对新生儿采用常规方法分离金黄色葡萄球菌,随机挑取住院成人对照（考虑标本科室分布、来源、时间等因素）,用全自动微生物分析仪进行菌种鉴定和药敏试验,采用苯唑西林纸片扩散法检测耐甲氧西林金黄色葡萄球菌（MRSA）,统计分析用2χ检验。结果：新生儿共分离金黄色葡萄球菌70株,年龄为3~36天,所有分离株对替考拉宁和万古霉素均敏感。新生儿分离的金黄色葡萄球菌β-内酰胺酶全部阳性,MRSA发生率为68.6%,对庆大霉素、左旋氧氟沙星、利福平、复方新诺明、四环素的耐药率分别为11.4%、8.6%、3.0%、20.0%、22.9%,均显著低于成人分离株的耐药率（P〈0.01）。结论：新生儿感染金黄色葡萄球菌的耐药率普遍低于成人的耐药率。Objective：To investigate the prevalence and drug resistant patterns of Staphylococcus aureus in neonates. Methods： The isolates of Staphylocdccus aureus were isolated from neonates based on clinical information, the samples of compared adluts group were taken randomly which were considered all kinds of factors including distribution, orgins and time, etal. The identification and susceptibility test of the isolates were determined by Automated Microbiology Analyzer Disk - diffusion was used for detecting methicillin- resistant Staphylococcus aureus, MRSA.x^2 test was made to identify the significance of difference. Results ：70 strains were found from neonates between 3 - 36 days old, all isolates were susceptible to vancomycin and teicoplanin, all isolates carried 13 - Lactamase, 68. 6% of isolates were referred as MRSA among neonates. The resistant rates of Staphylococcus aureus from neonates to gentamicin, levofloxacin, rifampicin, compound sinomin and tetracycline were 11.4%, 8.6%, 3.0%, 20. 0%, 22. 9% ,were all significantly lower than those of isolates from compared adluts group. Conclusion：The resistant rates of Staphylococcus aureus to many antibiotics from neonates are lower than those of isolates from compared adluts group. The drug resistant patterns of neonates are significantly different from adluts.
