摘 要:目的探讨16层螺旋CT三维重建技术在肋骨骨折中的应用价值。方法对80例胸部外伤患者行16层螺旋CT扫描检查,均经多平面重建(multiplanar reconstruction,MPR)与表面遮盖显示(surface shaded display,SSD)技术后处理重建获得了满意的三维重建图像,显示出肋骨骨折的部位、数量,并与胸部平片对照分析,判断16层螺旋CT三维重建诊断肋骨骨折的优缺点。结果80例中总肋骨骨折数为265根,肋骨骨折以4~10肋骨(85.7%)、腋段(52.8%)常见。胸部平片诊断了69例(86.3%)、231根(87.2%)肋骨骨折,16层螺旋CT发现了胸部平片漏诊的11例、34根(12.8%)肋骨骨折。胸部平片漏诊的肋骨骨折多为无移位或肋软骨、胸椎的骨折。16层螺旋CT三维重建诊断了79例(98.75%)、263根(99.2%)肋骨骨折。诊断肋软骨骨折7根。结论16层螺旋CT三维重建对肋骨骨折的诊断具有较高的临床应用价值。Objective To investigate the value of 16-slice helical CT 3D reconstruction in diagnosis of the rib fracture. Methods 16-slice helical CT scanning was performed in 80 cases with thoracic injury. Excellent 3D reconstructed images were obtained by using multiplanar reconstruction(MPR) and surface shaded display( SSD) technique. The position and number of rib fracture were observed on 3D images and compared with that of chest X--ray film in order to find the advantage and disadvantage in diagnosis of rib fracture by 16-slice helical CT 3D. Results Of the total 265 fractures of ribs in 80 cases, The majority of rib fractures occured in 4-10 rib( 85.7%) and armpit segments( 52.8%). 231 rib fractures( 87.2%) in 69 cases(86.3%) were displayed by chest plain film. 34 rib fractures(12.8%) in 11 cases were missed by chest plain film, most of those near the rib cartilage or thorax vertebra. 16 - slice helical CT 3D reconstruction displayed all fractures. 79 cases( 98.75 % ) , 263 ( 99.2 % ) rib fractures were detected by 16- slice helical CT 3D reconstruction,including 7 cartilage fractures. Conclusion 16-slice helical CT 3D reconstruction is of significant value in diagnosing rib fractures.
