摘 要:目的:利用64层螺旋CT血管成像探讨肾动脉和左肾静脉解剖变异的分类、分型及其临床意义。材料和方法:回顾性分析213例患者的64层螺旋CT肾血管影像资料,观察肾动脉和左肾静脉的解剖变异并进行分类、分型。结果:根据主肾动脉的数量将肾动脉的解剖表现分成两类并详细分型,ⅠA型(正常型)分别占病例数和肾脏数的62.9%(134/213)和77.9%(332/426),肾动脉变异分别占病例数和肾脏数的37.1%(79/213)和22.1%(94/426),其中ⅠB型、ⅠC型、ⅠD型和ⅠE型分别占9.6%(41/426)、10.8%(46/426)、0.2%(1/426)和0.5%(2/426);ⅡA型和ⅡB型分别占0.7%(3/426)和0.2%(1/426),未发现ⅡC型、ⅡD型和ⅡE型。性别、左右肾之间肾动脉变异发生率无显著性差异。同时发现2例环主动脉左肾静脉,5例左肾静脉汇合较晚以及1例左肾静脉回流入半奇静脉。结论:64层螺旋CT血管成像可清晰显示肾动脉和左肾静脉的解剖变异,不同的变异类型对指导肾脏手术、肾脏移植及相关疾病的诊断有重要的临床价值。Purpose: To discuss the category and significance of the variations of renal artery and left renal vein with 64-slice spiral CT angiography. Materials and Methods: The 64-slice spiral CT angiography of 213 patients were analyzed retrospectively. The variations of renal artery and left renal vein were observed and classified. Results : According to the number of the main renal artery, the anatomy of renal artery was classified 2 categories and typed detailedly. The incidence of category [ A (normal type) was 62.9% (134/213) of the patients and 77.9% (332/426) of the kidneys. The incidence of the variations of renal artery was 37.1% ( 79/213 ) of the patients and 22.1% (94/ 426) of the kidneys. In these patients, the incidence of category Ⅰ B, Ⅰ C, Ⅰ D, Ⅰ E, Ⅱ A and Ⅱ B was 9.6% (41/426) ,10.8% (46/ 426) ,0.2% (1/426) ,0.5% (2/426) ,0.7% (3/426) and 0.2% ( 1/426), respectively. But category II C, [I D and I]E were not found. There was no statistically significant difference in the incidence of the variations of renal artery in sex and the position of the kidney. Simulta- neously, 2 patients with circumaortic left renal vein, 5 patients with left renal vein converged late and 1 patient with abnormal confluens to the hemiazygos vein were found. Conclusion: 64-slice spiral CT angiography could show the anatomy of renal artery and left renal vein clearly. The different category of the variations shows significant clinic vaule in the guidance for kidney surgery, kidney transplantation and the diagnosis of some correlated diseases.
