批量伤员救治绿色通道的建立与实施

摘　　要：目的:各种自然灾害和人为灾害增加了突发事件的发生,可在短时间内造成大量伤员。绿色通道为患者在第一时间提供最适宜的救治,以最快速度为患者提供便捷优质服务,提高抢救成功率。文中探讨批量伤员救治中绿色通道建立的方法,优化批量伤员急救流程,提高急救水平。方法:对批量伤员的急救流程进行优化。根据伤员病情分为危及生命急重症、普通重症、非重症3级,救治时进入相应红、黄、绿3区;实行救护人员编组,分为指挥联络组、预检组、抢救组、清创治疗组、留观组、转运组、协调记录组7组;开放绿色通道,实行批量伤员编号制,危重伤员首诊负责制。结果:接诊23批次伤员共158例。治愈141例,死亡3例,院外死亡14例。结论:急救流程的优化在批量伤员的急救处理中发挥重要作用,建立有效的批量伤员救治绿色通道,能够使急救工作快捷、有序,忙而不乱,赢得抢救危重伤员的黄金时间。Objective：The occurrence emergency accidents was high in recent years and resulted in big mass easuaties in a short time. Green passage may provide the best care for the trauma patients. The aim of the study is to discuss the nursing management method of a green passage in the emergency care of mass casualty, and to optimize the emergency care quality of mass casualties in the hospital by the improvement of the green passage model. Methods：A modified emergency care model for mass casualties was designed. According to patients＇ severity of injury, they were classified into three categories, including critical, severe and mild cases. They were marked by red, yellow and green tags and assigned to cor- responding areas. The green passage was activated. All the patients were numbered and each critical patient was assigned a specific doctor. The staff were organized into command-communication group, triage group, resuscitation group, debridement group, observation group, conveying group and coordinating-recording group. Results ：23 batches of mass casualties with 158 patients in all were managed by our hospital from June 2004 to July 2008. There were 33 severe patients with TI score equal to or greater than 17. 141 patients were cured and discharged, but 3 died in hospital, and 14 died before reaching the hospital. Conclusion：Modification of emergency care model plays an important role in the management of mass casualties. Setting up an effective green passage of emergency care in mass casualty can make the emergency work easily, efficiently and systematically. It is the key point of success of rescue in mass casualties.
